背景

細菌感染は、同種造血幹細胞移植を受ける患者の疾患や死亡の最もよく見られる原因の 1 つである。シプロフロキサシン予防投与が推奨されているが、血清濃度や臨床経過についての情報が不足している。

目的

シプロフロキサシン濃度と予防投与の失敗の関係を調査すること。特に、異なる薬物動態指標(濃度時間曲線下面積[AUC 0–24 時間]対単一時間検体)は、転帰との相関が異なるかどうかに関して。

方法

この前向き観察的単一施設研究は、1,500 床の教育病院で実施され(2018 年 3 月から2019 年 3 月)、同種造血幹細胞移植を受ける成人患者でシプロフロキサシン予防投与を受ける 63 例が組み入れられた。血液検体を 3 つの採血時間に週に 2 回採取し(投与から 1、6、12 時間後)、高速液体クロマトグラフィーで測定した。発熱エピソードが始まることは、シプロフロキサシン予防投与の失敗が疑われることを示すとした。血液培養が陽性であること(血流感染症[BSI])は、予防投与の失敗が確認されたことを示すとした。

結果

63 例の患者のうち 7 例が死亡し、生存者と比べて、平均シプロフロキサシン濃度に有意差はなかった。発熱エピソードを経験した患者(54/63)は、生存確率が 13%(95%信頼区間[CI]4 ~ 22%)低かった。シプロフロキサシンの最初のエピソード 58 件から、3 つの値のセット(三つ組)が合計で 225 個得られた。グラム陰性菌(GNB)による BSI(GNB-BSI)に先行した三つ組は、平均の AUC0–24時間が低かったが、単一時間検体の指標は同程度であった。AUC 0–24 時間が 21.61 mgh/L 以下だと、GNB-BSI のオッズが 4 倍高い結果となった(調整後のオッズ比 3.96、95%CI 1.21 ~ 13.00)。これらの結果は、投与経路、患者の人口統計学的特性、検体採取のプロトコール逸脱とは独立していたので、GNB-BSI イベントではシプロフロキサシンへの曝露が減少していたことを示している。

結論

複数回の検体採取によりシプロフロキサシン濃度をモニターすることは、同種造血幹細胞移植に関連する細菌感染を減少させるのに役立つ可能性がある。因果関係を調べるためには、更なる解析が必要である。
