背景

2021 年に、エムポックス クレード IIb 感染成人患者が、ナイジェリア旅行からの帰国 14 日後、症状発現から 12 日後に、英国のHigh Consequence Infectious Diseases(HCID)Network 内の教育病院に入院した。その後、家庭内接触者 2 例(成人 1 例、小児 1 例)が症状を発症してエムポックス感染が確認され、管理のために同じ HCID の病棟に入院した。

方法

接触頻度が高い表面と低い表面の環境スワブを採取した。これには、最終除染の前後におけるサンプリングが含まれた。

結果

全体で、定量的 PCRにより解析したサンプル 222 個(32.4%)中 72 個でエムポックスウイルスが検出された。活動性感染症例が入院している病室から採取され、除染前後の結果が得られたサンプルのうち、65%が除染前、5%が除染後に陽性であった。無症状の接触者および回復症例が滞在する特別室についても環境サンプリングを行い、滞在中および除染前にサンプル 43 個中 9 個(20.9%)で陽性を認め、除染後はサンプル 27 個のうち 1 個(3.7%)が陽性であった。環境スワブにおける陽性率は、除染前(14 ~ 100%)および除染後(0 ~ 15%)で症例間でばらつきがあった。除染後、qPCR 陽性サンプルは減少し、依然として陽性であったサンプルでは検出された核酸レベルが低下した。qPCR 陽性サンプル 72 個中 13 個でウイルスの分離を試み、生きたウイルスがサンプル 2 個で同定された。これらのサンプルのうち 1 個は、活動性感染が認められた人のいない特別室で採取された。

結論

本研究は、感染予防・制御策について、清掃プロトコールの有効性を含めて評価する上での、エムポックス症例周囲の環境サンプリングの有用性を確立するものである。

監訳者コメント

mpox 病室環境の実測データとして除染前後を比較した点は感染対策上の実用性が高い。qPCR 陽性と感染性ウイルスの乖離があり、環境汚染の臨床的リスク評価には慎重な解釈が必要である。
