インドの公共部門の 3 次医療病院における感染予防・制御プログラムの準備についての多施設共同研究:低および中所得国からの見方★

背景
医療関連感染(HAI)は、低および中所得国における公衆衛生上の主要な懸念の 1 つである。それは、感染予防・制御(IPC)対策の実施に一貫性がないことと関係していて、そのため、抗菌薬耐性や患者への害のリスクが増加する。この研究は、インドの 3 次医療病院における IPC プログラムの準備と実施について観察することを目的とした。
方法
All India Institute of Medical Sciences が、インドの 15 の州と連邦直轄領の 31 の医療機関で、感染制御の専門家を対象とした多施設共同観察横断研究を実施した。IPC の 6 つのドメインであるプログラム、ガイドライン、教育、サーベイランス、モニタリング、インフラストラクチャーを対象とした妥当性が確認されている質問票を使った調査を、パンデミックに対する備えのための IPC についてのトレーナープログラムのトレーニング中に実施した。
結果
調査した施設の大部分(97%)は、公共部門の病院で、大多数(60%)は 500 床未満であった。IPC 委員会には、臨床の代表者がいたが、疫学者を含んでいたのは 58.1%のみであった。HAI のトレーニングは 74.5%の施設で提供されていて、61.4%には抗菌薬の方針があったが、基準に照らして評価した HAI のデータがあったのは 27.5%のみであった。サーベイランスおよび緊急時への備えでは差がみられ、53.3%は隔離室の存在を報告し、48.9%には患者急増時の計画があった。IPC の標準化された業務手順書は利用できるが、生物学的脅威に対する備えについては改善が必要である。
結論
IPC の枠組みは、公共部門の病院で強力になってきているが、サーベイランス、基準に照らした評価、人的資源、アウトブレイク中の患者急増に対する備えについては改善の機会が存在している。パンデミックに対する備えにおいて、これらのギャップを埋め、IPC をシステム全体で確実に有効なものとするには、病院のレベルで、焦点を合わせた改善と方針に基づく措置が必要である。
