急性呼吸器ウイルス感染症を有する参加者の咳嗽時に検出される空中浮遊粒子中の生存ウイルス★

目的
本研究は、呼吸器ウイルスに感染した参加者の自発的な咳嗽時に産生される排出粒子中の空中浮遊ウイルスの生存について評価することを目的とした。
方法
新規技術を利用して、多様な呼吸器感染症を有する患者を募集し、リアルタイム PCR 解析および培養分析を実施した。病院を受診して PCR で呼吸器ウイルス感染症陽性であった参加者 13 例の咳嗽後に、周囲の相対湿度下において呼吸器粒子を空中浮遊状態で平衡状態に達するまで定常化させた。
結果
鼻咽頭スワブのリアルタイム PCR 解析により以下の単独感染が同定された:A型インフルエンザウイルスが 5 件、RS ウイルスが 3 件、ライノウイルスが 1 件、パラインフルエンザウイルス-3が 1 件、およびパラインフルエンザウイルス-4 が 1 件。また、2 件の共感染が同定された:RS ウイルス/A型インフルエンザウイルスが 1 件、および RS ウイルス/ヒトコロナウイルス HCoV-HKU1 が 1 件。リアルタイム PCR 解析によりウイルスが同定されたのは、参加者 13 例中 9 例(69%)から採取された咳嗽粒子中であり、生存ウイルスが回収されたのは参加者 13 例中 7 例(54%)であった(A型インフルエンザウイルス[1 例]、RS ウイルス[4 例]、パラインフルエンザウイルス-3[1 例]、HCoV-HKU1[1 例])。RS ウイルス感染症の参加者はインフルエンザの参加者よりも多くの呼吸器粒子を排出しており、中央値で 1.07 × 104 ± 19.9 × 103 個/L であった(P = 0.044)。
結論
本研究から、多様な生存呼吸器ウイルスが検出されることが示され、例えば放置した咳嗽粒子においてパラインフルエンザウイルスおよび RS ウイルスに関する新たな洞察が得られ、また空気を介した伝播のリスクが浮き彫りにされている。
監訳者コメント :
空気を介する感染経路は古典的には粒子が 5μm より大きいか小さいかによって、飛沫感染、空気感染に分けていたが、近年は粒子のサイズで区切る分類は止める方向に議論が進んでいる。本研究で検出されたウイルスは古典的には飛沫感染に分類されるが、空気中でも生きたウイルスが確認されており、空気を介する感染経路での議論において参考となるものである。
