背景

第四級アンモニウム化合物(QAC)を配合した手指消毒用ワイプなどの手指衛生用代替製品は、携帯が容易で肌にやさしいため、ますます多く市販されるようになっている。

目的

QAC ワイプ、手洗い、および過酸化水素配合擦式アルコール製剤(ABHRs)について、タンパク質を含む有機物質で汚染された手指から微生物を除去する有効性を比較すること。

方法

自己対照クロスオーバー試験でボランティア 15 名を募集し、標準化された手指衛生トレーニングを行った。参加者には以下の 3 種類の介入法のトレーニングを行った:(1)非抗菌石けんおよび流水による 60 秒間の手洗い、(2)70%エタノール+ 0.2%過酸化水素による擦式製剤の 30 秒間の擦り込み、および(3)QAC ワイプによる 60 秒間の手指ワイプ。両手を、黄色ブドウ球菌(Staphylococcus aureus)(1.0 ~ 9.0 × 106 cfu/mL)で汚染させ、3%ウシ血清アルブミンを添加した。微生物の量を、手指当たりの対数減少値(log10 cfu/hand)を用いて 3 つの領域(手指、手掌、および手背)で定量した。

結果

QAC ワイプの有効性は、3 つの領域すべての細菌除去において石けんと同等であった一方、その有効性は擦式製剤より有意に優れていた。ワイプと石けんは、手背では手指よりも有効性が有意に低かった(P < 0.001)。手の領域における有効性の順番は、手指>手掌>手背の順であり、全体的な介入の有効性の順番は、石けん=ワイプ>擦式製剤の順であった。

結論

QAC ワイプは、有機物汚染の条件下で 60 秒間のワイプ手順プロトコールで用いた場合、その有効性は石けんによる手洗いと同等である。手指擦式製剤は、タンパク質の多い環境では有効性が有意に低いことが示される。

監訳者コメント

「有機物存在下での手指衛生」という、臨床現場において極めて現実的な状況を扱っている点は興味深い。アルコールはタンパク質存在下で効果が低下しやすいことが知られており、有機物存在下で石けん = QAC ワイプ > ABHR という結果が示されたことは理論的にも妥当である。
また、手背において有意に効果が低下したという所見は、感染対策教育の観点からも重要な示唆を与える。本研究は、「目に見える汚染がある場合は流水と石けんによる手洗いを行う」という既存のガイドラインを支持する内容である。
さらに、QAC ワイプは芽胞形成菌やノロウイルスに対しては効果が限定的である可能性があるものの、災害医療や在宅医療など、水の確保が困難な環境においては代替手段となり得る。
