背景

呼吸器ウイルスの院内伝播は病院ケアの重大な破綻の原因となるが、病棟における空間的な伝播動態については十分に解明されていない。

方法

SARS-CoV-2 伝播と高齢者病棟におけるベッド配置との関係を定量化するため、数値流体力学(computational fluid dynamics)によるシミュレーションをウイルス伝播の疫学的再構築に組み込んだモデルを開発した。

結果

数値流体力学シミュレーションのデータから、距離が 1 m 離れるごとに感染宿主への曝露が約 40%減少するパターンが認められ、患者を別室にした場合はさらに 4 倍減少した。しかし、統計学的推定の結果から、病棟で同定された伝播イベントのうち、本モデルで説明できたのは 72%(95%信頼区間 45% ~ 96%)のみであることが示唆された。他の伝播イベントは、本シミュレーションモデルとは一致しない遠い距離の場合に発生しており、このことから院内伝播において、設備の共用またはスタッフを介した伝播などの、距離に依存しない伝播メカニズムが重要な役割を果たすことが示唆される。

結論

本研究の結果から、距離を離すことで伝播リスクが低減される一方、個室の使用などの感染予防・制御戦略はアウトブレイクの予防には不十分である可能性が示唆される。包括的な院内伝播予防のアプローチにより、生じうる複数のウイルス伝播経路を対象とすることが必要である。

イギリスからの報告。SARS-CoV-2 伝播リスクの軽減には距離を離すことが重要である一方、それだけでは不十分で包括的なアプローチな必要であることが改めて認識できる。
