A systematic review and meta-analysis of outbreaks in neonatal intensive care units

背景

新生児集中治療室(NICU)は、医学的介入を緊急に必要とする新生児に専門医療を提供するように設計されている。しかし、これらの環境が感染の拡大を促進し、アウトブレイクにつながることもある。本レビューでは、20年間に報告された NICU に関連したアウトブレイクを解析する。

方法

PubMed、Scopus、Web of Science、Google Scholar および Outbreak Database を含む電子データベースの検索を行い、2004 年 1 月から 2024 年 10 月までに NICU で発生したアウトブレイクを報告した研究を特定した。データをテーマ別に統合し、記述的要約、表および図により示した。変量効果メタアナリシスを行い、発生率および死亡率を統合した。

結果

特定された記録 4,853 件中、119 件の研究が選択基準を満たし、42 か国における 122 件のアウトブレイクの記録が含まれた。細菌がアウトブレイクの 80.3%の原因であり、主な病原体は肺炎桿菌(Klebsiella pneumoniae)およびセラチア・マルセッセンス(Serratia marcescens)であった。主なリスク因子は早産および低出生体重であり、医療従事者の手指が主な伝播経路として特定された。

結論

NICU におけるアウトブレイクに至る感染症には、医療従事者との強い関連が認められる。有効な感染予防戦略に関するスタッフの教育および厳格な感染予防・制御戦略の実施が、今後のアウトブレイクを予防し、患者安全性を向上させるためにきわめて重要である。

監訳者コメント

日本における NICU でのアウトブレイクに至る医療関連感染では、MRSA 感染症が代表的であろう。NICU では、呼吸管理をはじめとして患児に全身管理が必須であり、医療従事者によるケア度も高い。このため、一度感染源となる病原菌が NICU 内で広がると蔓延しやすい。国際的な文献ベースでは、NICU 内では肺炎桿菌(Klebsiella pneumoniae)およびセラチア・マルセッセンス(Serratia marcescens)が代表的であったという。
