集中治療室における院内感染起因病原体の遺伝的多様性と生存期間の相関 ★

ヒト宿主の外側である病院環境の中では、細菌の生存能力に相違がある。生存しながら感染性を維持する菌種は、伝達の機会がより多くなり、病院内での適応性に優れている。この適応性の格差は、病院内の細菌集団における遺伝子構成を変化させ、数種の繁栄するクローンの相対的優位をもたらすと考えられる。この研究の目的は、集中治療室で前向きに分離された様々な菌種について、頑強さ(環境生存能力)と遺伝的多様性によるクローンの優位性との潜在的な相関関係を調べることにある。文献検索を行い、集中治療室で最も重要な病原体(黄色ブドウ球菌、Enterococcus属、Acetinobacter baumannii、緑膿菌、Enterobacter属、大腸菌、Klebsiella pneumonia、およびStenotrophomonas maltophilia)についての平均環境生存期間を同定した。これらの集中治療室内の病原体における遺伝子的多様性を決定するために、中等度の院内感染率をもつ5カ所の集中治療室で、18カ月の前向き研究を実施した。すべての臨床分離株を集め、高い識別能力をもつDNAフィンガープリンティング法を用いて特定のクローンの同定を行った。それぞれの菌種における多様性指数は、識別可能な遺伝子タイプの型数を菌株数で除して算出した。生存期間とそれぞれの病原体の多様性指数との相関は、ノンパラメトリック法により検定した。文献検索によって21報の研究を特定したが、関連性が認められたものは、わずか2報のみであった。これらの研究では、生存期間中央値の範囲は1.5日(緑膿菌)から60.0日(Enterococcus faecium)であった。集中治療室における前向き研究の期間に、1,264株の病原体を調査したところ、単純多様性指数の範囲は、49.1(Enterococcus faecalis)から89.9(大腸菌)であった。生存期間とそれぞれの病原体の多様性指数には、相関が認められた(相関係数0.821、P=0.024)。環境中の生存能力は、集中治療室における院内病原体の生物学的適応性に寄与する重要な因子であろう。感染対策では、今回の知見を考慮すべきである。
