背景

アシネトバクター・バウマニ(Acinetobacter baumannii)感染症は、世界中で医療の重大な問題となっているものの1つである。

目的

多剤耐性(MDR)および超多剤耐性(XDR)A. baumannii 感染症患者に対するコリスチン単独療法をコリスチン併用療法と比較すること。

方法

このシステマティックレビューおよびメタアナリシスでは、5 つの情報源(Cochrane Library、Google Scholar、Web of Science、Scopus、PubMed)と 2 つのレジストリ(Clinicaltrials.gov、ICTRP)を使用した。

結果

合計22 論文(無作為化対照試験[RCT]9 件、観察研究13 件)が解析に組み入れられた。組み入れられた患者は、コリスチン単独療法群例 1,271 例、コリスチン併用療法群 1,782 例で併用の組み合わせは、カルバペネム併用 909 例、スルバクタム併用 162 例、リファンピシン併用 145 例、チゲサイクリン併用 111 例、その他 455 例であった。全研究群で死亡率は高く、コリスチン―カルバペネム併用群で5 ~ 90%、コリスチン―スルバクタム併用群20 ~ 88%、コリスチン単独療法群21 ~ 87%であった。3 件の RCT におけるコリスチン単独療法対コリスチン―カルバペネム併用療法の死亡率に統計学的有意差はなく(オッズ比、0.65、95%信頼区間、0.28 ~ 1.52、P = 0.32)、4 件の観察研究におけるコリスチン単独療法対コリスチン―スルバクタム併用療法でも有意差は認められなかった(1.48、0.85 ~ 2.56、P=0.16)。

結論

これまでに実施された最も網羅的なシステマティックレビューおよびメタアナリシスによると、MDR および XDR A. baumannii 感染症患者の死亡率は、コリスチン単独療法およびコリスチン併用療法のいずれでも高く、両者間に統計的有意差は認められなかった。

監訳者コメント

日本でも多剤耐性アシネトバクター感染症は重要な院内感染問題である。本研究結果は、コリスチン併用療法が単剤療法より優れているという明確なエビデンスがないことを示しており、日本の感染症治療ガイドライン策定や抗菌薬適正使用推進に重要な示唆を与える。
