飲用水配管システムは抗菌薬耐性病原体が定着しやすい場所(hot spot)である★★

背景
飲用水配管システムにおける抗菌薬耐性病原体は、重大であるにもかかわらず十分に評価されていない公衆衛生上の脅威である。
方法
本研究は、オーストラリアの病院および住居の飲用水およびバイオフィルムのサンプルを対象に、主要な抗菌薬耐性の脅威である、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)、カルバペネム耐性緑膿菌(Pseudomonas aeruginosa)、およびカルバペネム耐性アシネトバクター・バウマニー(Acinetobacter baumannii)の拡がり状況を明らかにするために qPCR(※qPCR は quantitative PCR すなわち定量 PCR のこと) および培養をベースとした方法を用いた初の研究である。
結果
少なくとも 1 種類の標的病原体について qPCR 陽性であったのは、住居の水およびバイオフィルムのサンプルでは 73%であったのに対し、病院のサンプルでは 38%であり、また住居の配管システムの 45%には少なくとも 2 種類の標的病原体が定着していた。緑膿菌、A. baumannii、および黄色ブドウ球菌について qPCR 陽性が認められたのはそれぞれ、水およびバイオフィルムのサンプルの 37%、22.3%、および 21.7%であった。培養法を用いた場合、緑膿菌、A. baumannii、および黄色ブドウ球菌について陽性が認められたのはそれぞれ、サンプルの 10%、8%、および 7%であった。これらのうち、緑膿菌および A. baumannii の培養分離株のそれぞれ 29%および 28%がカルバペネム耐性、黄色ブドウ球菌の培養分離株の 54%が MRSA であると同定された。排水管のバイオフィルムには、抗菌薬耐性の A. baumannii、黄色ブドウ球菌、および緑膿菌の定着が最も多く認められた。バイオフィルムサンプルでは、カルバペネム耐性遺伝子として blaNDM-1、blaOXA-48、blaKPC-2 および blaVIM などが認められたが、緑膿菌については陰性であったことから、配管のバイオフィルムは細胞外 DNA(eDNA)のリザーバの役割を果たしていることを示している。
結論
本研究の結果は、配管のバイオフィルムが多様な抗菌薬耐性病原体の定着しやすい場所として重要な役割を果たしており、とくに在宅医療環境では、脆弱な集団におけるリスクを増大させることを示している。本研究は、飲用水配管システムにおける抗菌薬耐性のリスクを低減するために、サーベイランスの強化と対象を絞った介入が喫緊に必要であることを強調している。
監訳者コメント:
飲料水配管システムにおける 薬剤耐性(AMR)の潜在的リスクを評価したもので、病院と比較すると一般家庭の住居において、耐性菌の検出率が高く、耐性菌のリザーバとしての役割を担っている可能性がある。飲料水配管システム(特にバイオフィルムを形成した配管)は見過ごされがちな AMR のホットスポットであり、在宅での医療の増加を考えると、このまま看過できない状況である。住宅配管における AMR 対策と監視の強化は、今後の在宅医療のみならず、在宅から入院してくる患者による AMR の持込も考慮すると、感染予防戦略における重要なエレメントとなるであろう。
