背景
チャンネルを洗浄するブラシは、内視鏡の再処理にとって重要な道具の 1 つである。
目的
様々な素材で作られている洗浄ブラシの軟性内視鏡の生検チャンネルからの有機物の除去における有効性を調べること。
方法
合計で 168 本の洗浄された胃内視鏡を、4 つの異なる種類の洗浄ブラシの特異的な特徴に基づく 6 つのグループ(A、B、C1、C2、C3、D)に無作為に分けた。12%のウシ血清と 106 単位の大腸菌(Escherichia coli)を含んだ懸濁液を、各内視鏡の生検チャンネルに注入し、確実に十分に汚染させた。5 分のインキュベーション期間の後、実験のプロトコールに従い、ブラッシングと検体採取を実施した。洗浄プロセスの有効性を、蛋白質検出検査とアデノシン三リン酸検査を使って評価した。
結果
金属の柄のある洗浄ブラシは、プラスチックの柄のある洗浄ブラシと比べて、軟性内視鏡から有機物を除去する性能が優れていることが示された(P < 0.05)。同様に、円柱状の DuPont ファイバーで作られた剛毛のあるブラシは、ストリップマイクロファイバーやキャタピラーポリオレフィンで作られたものと比べて、洗浄の有効性が優れていることも示された(P < 0.05)。洗浄ブラシを生検チャンネルに 4回通過させた方が、1 回や 2 回と比べて、良好な結果が達成された(100%)。
結論
洗浄の転帰および経済的な利益という観点からは、軟性内視鏡の再処理で、金属の柄と円柱状の DuPont ファイバーの剛毛のある洗浄ブラシの使用を、生検チャンネルにブラシを 4回通過させる洗浄法と組み合わせると、マイクロファイバー、ポリオレフィン、プラスチックで作られた洗浄ブラシの使用と比べて、有機物をより効果的に除去できる。
監訳者コメント:
内視鏡の洗浄ブラシの洗浄効果を比較検討した研究であるが、内視鏡の再処理においてマニュアルによるブラシ洗浄は必須の手技であり、洗浄ブラシの有機物除去効果を比較することで、ブラシ本体の材質が大きく洗浄効果に依存することがこの研究で明らかとなった。今後多くの知見が積み上げられ、最終的に内視鏡の洗浄消毒ガイドラインの中に「洗浄ブラシの種類」についても推奨度と条件などが言及されることを期待する。
