背景

抗菌薬耐性は、新たな脅威であり、カルバペネマーゼ産生腸内細菌目細菌(CPE)とバンコマイシン耐性腸球菌(VRE)がかなりの難問となっている。この研究では、入院患者における CPE/VRE の獲得と広まりを、全ゲノムシークエンシング(WGS)を使い耐性遺伝子とマイクロバイオームを分析し、疫学的なリスク因子や臨床的なリスク因子を探ることによって検討した。

方法

この後向き研究には、3 次病院の感染症科からの患者が組み入れられた。臨床情報および疫学情報とともに、毎週糞便検体を採取した。CPE/VRE が分離された患者は、耐性遺伝子の存在を評価するために WGS が行なわれた。入院時と退院時にマイクロバイオーム分析を実施した。WGS は、研究期間中に院内で採取されたすべての陽性検体についても実施した。

結果

102 例の参加者のうち、16 例(15.7%)は、入院時に CPE/VRE を保菌していた。入院時に CPE/VRE を保菌していることの重要なリスク因子は、最近の入院、手術、抗菌薬の使用等であった。入院の最初の 1 週間が、CPE/VREの獲得に極めて重要であり、認知症、中心カテーテルや尿路カテーテル、カルバペネムの使用が、有意なリスク因子であった。入院後に獲得された CPE/VRE は、病院で広まっている株と耐性遺伝子が共通であったが、入院時に存在していた CPE/VRE は、blaOXY-4-1optrA のような新規の遺伝子を持っていることが特徴であった。マイクロバイオーム分析では、種の豊富さに有意な差があり、退院時に多様性が減少していたことが明らかになった。

結論

感染症科への入院時に CPE/VRE を保菌している確率は高かった。厳格な感染制御策が、CPE/VRE の広まりを防ぎ、新規の耐性遺伝子の導入を防ぐには不可欠である。

監訳者コメント

日本でも CPE/VRE の院内伝播は重要な課題であり、特に長期療養型病院や地域医療機関からの転院患者における保菌率の高さが問題である。本研究が示すように、入院時スクリーニングと入院初期の厳格な感染対策が極めて重要である。日本では全ゲノム解析の実施体制がまだ限定的であるが、地域の基幹病院や感染症指定医療機関を中心に導入を進め、耐性遺伝子の動向監視体制を構築すべきと思われる。
