Risk factors for infection in patients undergoing hip replacement: a systematic review and meta-analysis

人工股関節置換術後の感染リスクを検討するためにメタアナリシスを実施し、手術部位感染症のリスク因子を特定した。PubMed/Medline、Embase および Cochrane Library データベースを系統的に検索するとともに、これまでのシステマティックレビューおよびメタアナリシスの参考文献一覧を検索した。ランダム効果モデルを用いて、検討した各因子の統合オッズ比（OR）を推定した。観察研究のエビデンスは、GRADE システムに基づいて高い質（クラス I）、中等度の質（クラス IIまたは III）、低い質（クラス IV）に分類した。データベース検索において、合計 25,383 報の候補論文が選択基準を満たした。タイトル、アブストラクトおよび全文を検討後、27 報を最終解析に組み入れ、股関節形成術を受けた患者 699,473例を含めた。高い質のエビデンスにより、BMI（≧30 kg/m²）（OR 2.16、95％CI 1.72 ～ 2.70）、男性であること（OR 1.39、95％CI 1.26 ～ 1.53）、手術時間（≧120分）（OR 1.72、95％CI 1.39 ～ 2.14）、米国麻酔科学会（ASA）クラス 3 以上（OR 2.05、95％CI 1.49 ～ 2.83）、糖尿病（OR 1.33、95％CI 1.23 ～ 1.43）および心疾患（OR 1.99、95％CI 1.48 ～ 2.68）が、より高い感染リスクに関連することが示されている。メタアナリシスでは、年齢および腎臓病は感染に関連しないことが明らかになった。結論として、本メタアナリシスでは、人工股関節置換術後の感染の有意なリスク因子として、BMI、男性、手術時間、心疾患、糖尿病および ASA クラスなどが特定された。
