背景

カンジダ・パラプシローシス(Candida parapsilosis)バイオフィルムは、殺生物薬に対するその抵抗性や休眠細胞の存在のために、臨床環境において大きな課題をもたらしている。本研究では、フルコナゾール耐性 C. parapsilosis 分離株が形成したバイオフィルムに対するアムホテリシン B、クロルヘキシジンおよびオルトフタルアルデヒドの有効性を評価し、またバイオフィルム抵抗性における休眠細胞およびカテーテル材量の役割を検討した。

方法

病院表面から得た C. parapsilosis 臨床分離株を、クロルヘキシジン(0.5%)およびオルトフタルアルデヒド(0.55%)に曝露した。最小発育阻止濃度(MIC)を、微量液体希釈法により定量した。バイオフィルムの殺生物薬感受性を、クリスタルバイオレットアッセイおよび総生菌数を用いて評価した。走査電子顕微鏡により、カテーテルの異なる材料上におけるバイオフィルム構造の変化を分析した。

結果

アムホテリシン B は高い有効性を示し、MIC および最小殺真菌濃度(MFC)が低値であった。クロルヘキシジンは中等度の有効性を示し、臨床分離株では標準株よりも MIC および MFC が高かった。オルトフタルアルデヒドは有効性が最も低く、MIC、MFC、および付着細胞の 80%の発育を阻止する最小濃度(SMIC80)はより高値であった。休眠細胞は分離株 CBL 1031 で確認された一方、分離株 17D、26E、および ATCC 90019 では SMIC80 に大幅な低下が認められ、高い殺生物薬感受性が示された。材料がポリテトラフルオロエチレンの場合は殺生物薬曝露後にバイオフィルムの有意な減少が示された一方、高密度ポリエチレンの場合はバイオフィルムの抵抗性がより高く、残存生菌数も多かった。走査電子顕微鏡により、材料特異的なバイオフィルム減少が認められ、ポリテトラフルオロエチレンと比べて、高密度ポリエチレンおよびシリコンコーティングされたラテックス上ではより大きな損傷がバイオフィルム構造に認められた。

結論

本研究の結果から、医療環境においてバイオフィルムに関連する感染症の低減において、洗浄プロトコールの最適化、適切な医用材料の選択、ならびに休眠細胞への対処の重要性が強調されている。

C. parapsilosis はバイオフィルムを形成し、血管留置カテーテルなどの医療器材・機器や病院環境において生存し、院内感染の起炎菌として日常的に遭遇する真菌である。消毒薬への感受性とカテーテル素材の違いによる本真菌の動態を研究することで、より効果的な予防対策と治療戦略を考えることが可能となる。
