背景

頭蓋内脳神経外科手術後の手術部位感染(SSI)は内因性および外因性の因子に由来し、病的状態、死亡および社会経済的コストの増加と関連している。手術室の換気にモバイル型層流(MLAF)ユニットを追加すると細菌を保有する粒子が減少するが、MLAF ユニットが SSI の発生に及ぼす影響、ならびにこれの他の因子との併存については明らかでない。本研究は、MLAF ユニットを用いた先行研究の継続研究である。

目的

MLAF ユニットの使用期間における、頭蓋内脳神経外科手術後の SSI に関連する因子を探索すること。

方法

本後向き登録研究は探索的デザインであった。頭蓋内脳神経外科手術を受けた患者の電子カルテを、45 項目から成るプロトコールを用いて SSI の徴候または症状について後向きにレビューした。人口統計学的変数、患者特異的および手術手技特異的変数、ならびに外因性の因子および SSI に関する変数(SSI の深さ、細菌の種類、感染までの期間、および治療に関するもの)を収集した。データの解析は、単変量および多変量ロジスティック回帰を用いて行った。

結果

SSI は対象患者 800 例中 55 例(6.9%)で発生した。単変量および多変量解析では、入院中の遠隔部位感染のみが SSI リスクと関連した(オッズ比 2.02、95%信頼区間 1.07 ~ 3.82、P = 0.031)。表在性 SSI が最も多く、感染までの期間中央値は 18 日であり、原因微生物で最も多かったのは黄色ブドウ球菌(Staphylococcus aureus)であった。

結論

臨床的な文脈では、入院中に遠隔部位感染が継続している脳神経外科患者に対しては考慮することが推奨される。手術に関連する感染予防の分野において、さらなる周術期研究が必要である。
