A scoping review of disinfection strategies for carbapenemase-producing Enterobacterales in hospital water systems

背景

カルバペネマーゼ産生腸内細菌目細菌（CPE）は世界的に健康危機をもたらしている。従来の抗菌薬や多数の消毒薬に対するそれらの耐性は、治療やリスク軽減の医療費を増大させる。病院の給水システムは CPE のリザーバーであり、的を絞った感染予防・制御（IPC）戦略が必要である。

目的

病院の給水システム内の CPE に対する消毒戦略に関する現在のエビデンスを、実際の適用、課題および IPC 統合に焦点を当てて評価し、集約した。

方法

2014 年 12 月 1 日から 2024 年 12 月 31 日までの研究を対象に、Preferred Reporting Items for Systematic Reviews and Meta-Analysis（PRISMA）ガイドラインに従って、スコーピングレビューを実施した。スクリーニングした記録 1,188 件のうち、22 件が選択基準を満たした。テーマ分析により、結果を化学的戦略、物理的戦略および統合的戦略に分類した。

主な結果

次亜塩素酸ナトリウムおよび過酸化水素は汚染を一時的に減少させたが、バイオフィルムに対する効果は低かった。一方、酢酸は定期的な使用により一貫した有効性を示した。四級アンモニウム化合物は有効性が証明されたが、標準化されたプロトコールが必要であった。蒸気洗浄や排水管カバーなどの物理的介入は汚染を減少させ、配管の変更はバイオフィルム形成を最小限に抑えた。汚染されたシンクの撤去や、水を使用しない環境の採用などの、資源集約的な方法も有効であった。化学的消毒、インフラの改良およびシンクプロトコールの強化を組み合わせた統合的アプローチが最も持続的な成果を示し、紫外線やバイオフィルムを標的とする発泡剤などの新規技術は、かなり有望であった。

結論

病院の給水システムにおける有効な CPE 制御には、バイオフィルムに保護されたリザーバーに対処する集学的な作業が必要である。特定の状況に合わせて作られた、実績のある消毒薬の定期的使用を、インフラの改良および IPC 戦略の包括的な実施と組み合わせることにより、最も有望な結果が得られる。手順を標準化し、抗菌薬耐性のさらに幅広い脅威を緩和するためには、新規技術とガイドラインの改訂が不可欠である。
