経静脈栄養におけるカテーテル関連血流感染症に対するインラインフィルターの影響およびリスク因子★

背景
カテーテル関連血流感染症(CRBSI)は、医療関連感染の約 15%、院内菌血症の最大40%を占める。インラインフィルターは、輸液ラインから、粒子、空気、微生物を除去する目的で広く使用されているが、感染予防における有効性は、依然として不明瞭である。本研究では、経静脈栄養を受ける患者を対象に、インラインフィルター使用とCRBSIの関連、およびリスク因子を評価した。
方法
2020 年 6 月から 2025 年 5 月の間に三重大学医学部附属病院で経静脈栄養を受けた成人を後方視的に調査した。18 歳以下および、エルネオパ®、フルカリック®、ビーフリード®以外の経静脈栄養製剤を使用した患者は除外した。インラインフィルター使用群と非使用群で CRBSI の発生率を比較し、χ2 検定、対応のない t 検定、決定木解析を用いてリスク因子の階層性を検討した。
結果
862 例(フィルター群 294 例、非使用群 568 例)が対象で、両群のベースラインは同等であった。CRBSI の発生率は、インラインフィルター群(13.6%)で、非使用群(26.8%)と比較し有意に減少していた(P < 0.001)。経静脈栄養の投与期間の延長、インラインフィルター不使用、集中治療室(ICU)への入室が、CRBSI の有意なリスク因子であった。決定木解析では、インラインフィルターの使用、投与期間、ICU への入室が、重要な決定因子として特定され、ICU への入室と投与期間延長の組み合わさった場合が、最も高リスクの分枝を形成した。
結論
インラインフィルターは、CRBSI の減少と関連していた。これは、インラインフィルターが注入に関係する汚染の予防において利益をもたらす可能性を示している。しかし、ICU 入室と投与期間延長が、依然として強い予測因子であった。これは、高リスクの状況の場合、フィルターのみでは不十分であることを示している。全体として、フィルターは、より幅広い感染制御対策の補完となる可能性があるが、有効性と費用対効果を確認するための前向き試験が更に必要である。
