3 次病院単施設における腸球菌による院内血流感染症の臨床特性および微生物学的特性:2007 年から 2019 年の後向き研究★

背景
腸球菌は院内血流感染症(BSI)を引き起こす重要な病原体であり、多くの種類の抗菌薬に対して自然耐性を有するため、適時の経験的抗菌薬療法を適切に行うことができない。
目的
腸球菌による院内 BSI の臨床的および微生物学的特性について分析すること。
方法
Xiamen University Zhongshan Hospital における腸球菌による院内 BSI 患者の臨床的および微生物学的特性について、後向き症例対照研究により検討した。すべての患者情報を、電子サーベイランスシステムにより収集した。
結果
13 年間において、症例計 199 例が腸球菌による院内 BSI と同定された。BSI の発生率には、0.21%から 0.81%の範囲でばらつきがあった。病棟別の分布でみると、肝胆手術において腸球菌による BSI が最も多かった。腹腔内疾患(オッズ比[OR]3.36、95%信頼区間[CI]2.15 ~ 5.27、P < 0.001)、化学療法歴(OR 4.37、95%CI 2.06 ~ 9.25、P < 0.001)、および尿道カテーテル留置(OR 2.34、95%CI 1.52 ~ 3.61、P < 0.001)が、腸球菌による院内 BSI 獲得のリスク因子であった。バンコマイシン耐性腸球菌(VRE)およびリネゾリド非感受性腸球菌株は検出されなかった。
結論
腹腔内疾患、化学療法、および尿道カテーテル留置のいずれかの既往を有する患者では、腸球菌による院内 BSI のリスクが高くなる。腸球菌株は、依然として一般的に用いられている抗菌薬に対する感受性を有していた。
