目的

本研究は、末梢挿入式中心静脈カテーテル(PICC)の皮下トンネル作成による挿入が、中心ライン関連血流感染症(CLABSI)の発生を減少させられるか否かを評価することを目的とした。

方法

3 次病院 5 施設において多施設共同非盲検無作為化対照試験を実施した。PICC を必要とする成人入院患者を、中央集中型のウェブベースによるコンピュータ生成層別化無作為化を用いて、従来式の PICC(conventional PICC:cPICC)群またはトンネル式 PICC(tunnelled PICC:tPICC)群に 1:1 の比で無作為化した。CLABSI 発生率の群間比較は修正 intention-to-treat 集団で行った。安全性として出口部感染症の発生率および出血に関連するカテーテル抜去を含めた比較も行った。本試験は、Clinical Research Information Service of Republic of Korea(KCT0005521)に登録された。

結果

2020 年 11 月から 2023 年 3 月にかけて、参加者 1,324 例を登録し、tPICC(N = 662)または cPICC(N = 662)に無作為に割り付けた。本研究は、コホートの CLABSI 発生率が推定よりも低かったために早期に中止され、したがって最初の被験者数 1,694 例では、本研究において CLABSI 発生率における差を検出するための検出力が不足することになった。CLABSI は、tPICC では11,071 カテーテル日において 651 例中 13 例で発生したのに対し(1,000 カテーテル日あたり 1.2)、cPICC では 11,141 カテーテル日において 650 例中 20 例で発生した(1,000 カテーテル日あたり 1.8)(発生率比 0.65、95%信頼区間 0.30 ~ 1.38、P = 0.30)。出口部感染症の発生率(tPICC で29 件、cPICC で36 件、P = 0.5)および出血に関連するカテーテル抜去(tPICC で 11 件、cPICC で 11 件、P = 0.99)には、2 群間で差は示されなかった。

結論

被験者数が不十分であったため、本研究では tPICC 群において cPICC 群と比較して統計学的に有意な CLABSI リスクの低下を示すことができなかった。両群において出口部感染症および出血の発生率は同程度であった。

監訳者コメント :

PICC の感染率は一般的に 3.4 ~ 7.4%と報告され、他の CVC に較べ、CLABSI の発生率が低いことがわかっている。また感染のリスク因子としては入院期間、ICU 入室、カテーテルルーメン数、化学療法の実施がある。皮下トンネル PICC では 1.6 ~ 3.0%と従来の PICC よりは低い。しかしながら、本論文では、従来法と皮下トンネル法による PICC のCLABSI の発生率を多施設ランダム化対照試験で実施されたが、当所の予想よりも発生率が両者において低かったため、統計的有意差を得るためのサンプルサイズが大きくなり中止となっているが、従来型と比較しても皮下トンネル作成による挿入時間は数分増える程度とのことである。さらなるデータの蓄積が待たれる。
