抗菌薬適正使用支援に関する情報提供のための複雑性理論の活用：スコーピングレビュー★

背景
ヒト医療研究において、複雑性理論はこれまで概念的なレンズとして使用されてきた。抗菌薬適正使用支援（AMS）は本質的に複雑な医療介入であるが、AMS において複雑さがどの程度活用されているかは、現時点では不明である。
目的
ヒト医療において、AMS に関する情報提供のために複雑性理論が活用されているとしたら、いかに活用されているか調査すること。
方法
スコーピングレビュー法。あらゆる医療施設および地理的地域において AMS との関連で複雑性に特に言及している実証的研究または方策について検討した。Cinahl、Cochrane Library、Embase、Medline、National Institute for Health and Care Excellence、PsycInfo、Scopus、Web of Science のデータベースを開始から 2020 年 6 月まで検索した。灰色文献や他のデータベースの検索には、EVIPNet、Google、Mednar、Proquest Theses、the World Health Organization Library of National Antimicrobial Resistance Action Plans を用いた。英語以外の論文は除外した。
結果
検索した論文 612 報のうち 8 報を本レビューの対象とした。研究デザインおよび地理的地域の不均一性が示された。3 件の介入研究は、病院（2 件）と長期ケア施設（1 件）で AMS を評価した。それ以外は非介入研究で、複雑性理論により概念化された AMS 対策を提示した。すべての研究において、AMS の妥当性および成功を確実にする手段として、研究者または政策管理者と対象集団との密接な関わりの重要性が強調された。
結論
複雑性理論から情報を得る AMS 研究は不足しており、指針となる理論として複雑性を用いる政策文書は特定できなかった。複雑性理論から情報を得る混合法は、複雑な介入として AMS を構築、実施、評価するために適切な方策となる可能性がある。
監訳者コメント ：
抗菌薬適正使用支援（AMS）という言葉は、だいぶ一般化したが、その AMS が定番化して適正化したかどうかは第三者評価方法の標準化が必要である。広く一般医療における AMS の普及には、ガイドラインや教育プログラムの運用が必要であろう。
