医療器材の再処理時における超音波発生器のエアロゾル発生による作業者のリスク:微粒子およびバイオバーデンの分析★

背景

再利用可能な医療器材および手術器具の再処理は、病院における安全な医療を保障する上で極めて重要である。医療器材再処理部門は、これら器材の洗浄、消毒および滅菌を担当している。以前の研究では、表面上のバイオバーデンと、患者の健康における関連リスクについて検討したが、医療器材再処理部門の作業者における職業的リスクについてはあまり焦点が当てられていない。

目的

カナダ・トロントにある Mount Sinai Hospital の医療器材再処理部門において、超音波滅菌水槽によるバイオエアロゾル曝露の可能性ならびに生成される粒子の濃度を調べること。

方法

アンダーセン式サンプラーを用いてバイオエアロゾルサンプルを採取し、細菌および真菌の培養を行った。粒子サンプルについては、光学式粒子サンプラーを用いて測定した。

結果

バイオエアロゾルの大部分は、低リスクの皮膚微生物叢および水系細菌から成っており、ミクロコッカス・ルテウス(Micrococcus luteus)およびブドウ球菌属菌が多かった。しかし、シトロバクター(Citrobacter)属菌およびアシネトバクター(Acinetobacter)属菌などの、有害な可能性のある細菌が検出された。同定された真菌属は、アスペルギルス(Aspergillus)属、クラドスポリウム(Cladosporium)属およびペニシリウム(Penicillium)属などであった。

結論

超音波洗浄によるエアロゾル発生は全般的に少ないようであったが、本研究により適切な個人防護具の重要性が強調されるとともに、医療器材再処理部門における換気およびさらなるエアロゾル発生源に関する今後の研究の必要性が示唆される。
