結核の負荷が中間の国における医療従事者に対する潜在性結核感染プログラムの効果★

背景

医療従事者における潜在性結核感染に対処することは、院内結核の伝播を予防するのに極めて重要である。我々は、結核の負荷が中間の国における 3 次病院の医療従事者に対する潜伏結核感染プログラムの効果を明らかにすることを目的とした。

方法

2013 年に、高リスクの診療科の医療従事者の毎年の検診とともに、新しく雇われた医師と看護師に対して、ベースラインの潜在性結核感染スクリーニングを実施した。潜在性結核感染のあった医療従事者には治療も提供した。2017 年以降は、潜在性結核感染の結果がなかった医療従事者全員が、潜在性結核感染について検査された。2013 年から 2020 年までの間の医療従事者における活動性結核の年間発生率を評価した。それに加えて、2020 年にこの病院に雇われた医療従事者における活動性結核の発生率を評価した。特に、2013 年から2018 年までの間に潜在性結核感染検査を受けていた人に焦点を当て、彼らの潜在性結核感染検査の結果と治療状況を検討した。

結果

医療従事者における活動性結核の発生率は、2013 年から 2020 年の間に有意に減少した。肺結核のリスクは 1 年当たり平均で10.2%減少した(95%信頼区間 1.0 ~ 19.0、P = 0.034)。2020 年にこの病院に雇われた医療従事者では、4,354 名が 2013 年から 2018 年までの間に潜在性結核感染検査を受けていた。そのうち927 名(21.3%)が検査で陽性であった。潜在性結核感染の治療を受けなかった 588 名のうち 9 名(1.5%)が、活動性結核を発症した。再度検査を受けた 1,285 名の医療従事者のうち 62 名(4.8%)が陽性転化し、治療を受けなかった 23 名のうち 1 名(4.3%)が活動性結核を発症した。治療を受けた人で活動性結核と診断された人はいなかった。

結論

潜在性結核感染プログラムは、医療従事者における活動性結核の発生率を有意に減少させた。潜在性結核感染スクリーニングと治療を、特に結核の負荷が高い国や中間の国では、実施すべきである。
