背景

発熱性好中球減少症は幹細胞移植の合併症としてよくみられる。

目的

耐性グラム陰性菌(拡張型βラクタマーゼ[ESBL]陽性、多剤耐性[MDR]緑膿菌[Pseudomonas aeruginosa])を保菌する発熱性好中球減少症患者における敗血症の頻度、ならびに保菌患者の第一選択抗菌薬について評価すること。

方法

本後向き研究は、2018 年 1 月から 2022 年 9 月に造血幹細胞移植を受けた患者のデータを分析した。病院情報システムからデータを抽出した。

結果

第一選択抗菌薬としてカルバペネムの選択は、AmpC 陽性/陰性の非保菌患者の 10.9%、ESBL 陽性の保菌患者の 31.5%、MDR 緑膿菌患者では 0%であった。MDR 緑膿菌を保菌する発熱性好中球減少症患者は、敗血症の罹患率が高かった(100%、P = 0.0197)。敗血症の病因分布は異なるようであり、ESBL陽性群ではグラム陰性桿菌が優勢を占めた(オッズ比 5.39;95%信頼区間 1.55 ~ 18.76、P = 0.0123)。MDR 緑膿菌定着が認められる敗血症の 100%が定着菌による敗血症であり(P = 0.002)、すべてが同種移植で(P = 0.0003)、死亡率は 33.3%(P = 0.0384)であった。ESBL 陽性の保菌患者の敗血症発生率は 25.9%(P = 0.0197)であり、敗血症症例の 50%が定着菌によるもので(P = 0.0002)、大半が同種移植であった(P = 0.0003)。

結論

本研究の結果から、MDR 緑膿菌の定着が認められる発熱性好中球減少症において、敗血症(この病態はほぼ例外なく定着菌に起因する)の有意なリスクが示された。加えて、ESBL 陽性菌保菌患者では、グラム陰性菌による敗血症のリスクがより高いことが確認された。

監訳者コメント

本研究が示すように、多剤耐性緑膿菌が定着した発熱性好中球減少症の患者はその多剤耐性緑膿菌による敗血症を引き起こすリスクが高いことを本研究が明らかにした。
