背景

医療関連感染において、緑膿菌(Pseudomonas aeruginosa)に対する懸念が増加している。緑膿菌は、重篤な基礎疾患のある患者にはかなりのリスクとなる。この病原体の抗菌薬耐性の特性と、バイオフィルムを形成する能力は、効果的な減少および消毒戦略を難しくしている。この難問に更に追加されるのが、アカントアメーバ属(Acanthamoeba)のような自由生活アメーバが、緑膿菌の検出、消毒、伝播において果たす役割である。緑膿菌は、アメーバの細胞内で生存が可能であり、このことが、検出能力を限られたものにし、高リスク領域への伝播を許している可能性がある。

方法/結果

我々は、スコットランドの機能的総合病院 1 施設において、アカントアメーバ属と緑膿菌の存在を、培養と分子的な方法でスクリーニングしたところ、4 か月にわたる期間中、数カ所、特に、床にある病室をつないでいる排水管内に存在していることが分かった。それに加えて、マイクロバイオーム分析では、アメーバが、環境スワブにマイクロバイオームシークエンシング技術を使うことでは容易に検出されなかったシュードモナス属(Pseudomonas)で主に構成される独特の微生物群集を含んでいることが明らかになった。病院の環境で、両方の微生物が一貫して存在していたことを示した後、我々は、研究室で、アカントアメーバと緑膿菌の関係を調べ、(i)アカントアメーバの増殖率は、緑膿菌のバイオフィルムが存在すると増加すること、および緑膿菌がアメーバの中で生き残ることと、(ii)過酸化水素主体の消毒薬は、アカントアメーバが存在すると、緑膿菌の分離株に対する効果が、この細菌を単独で培養するときよりも、有意に低下することを明らかにした。

結論

これらの研究結果は、アメーバや、その他の原生生物は、高リスク領域における緑膿菌の検出と存続に影響を与える可能性があり、減少戦略を実施する際に考慮すべきであることを示唆している。
