背景

リスクの高い内視鏡の手動洗浄に対するモニタリングの必要性は推奨されているのみならず、現行の内視鏡再処理ガイドラインではさらに強く推奨されている。本研究の目的は、大腸内視鏡および気管支内視鏡について最適な抽出量を確立すること、また、ChannelCheck 迅速検査における有効回収率を明らかにすることであった。

方法

陽性対照として用いた試験用汚染物は 20%脱線維素ウシ血を含有する ATS2015 とした。大腸内視鏡および気管支内視鏡のチャネル部品からの抽出物を評価して、最適な抽出量、ならびに蛋白質、炭水化物およびヘモグロビンを対象とした有効回収率の確立を目指した。

結果

試験を行った抽出量のうち、10 mL が大腸内視鏡と気管支内視鏡の両方において最適であった。有効回収率は、炭水化物では 91%、ヘモグロビンでは 83.7%、ならびに 蛋白質では 82.4%であった。

結論

これらの分析物に対する ChannelCheck 迅速検査の検出限界は、確立されているレベルを満たすか超えており、軟性内視鏡の適切な手動洗浄と対応している。

監訳者コメント

手動洗浄された内視鏡は、使用した水、環境や患者由来の有機物や微生物により汚染されており、洗浄後の内視鏡の 3 ~ 5%においても残渣が確認され、感染伝播のリスクは残っている。日常的に「感染伝播を防ぐに十分な」洗浄できたかどうかの評価が必要であり、ATP や培養によるモニタリングが実施されている。一方、本迅速キットはタンパク質、炭水化物およびヘモグロビンの残留を確認する、ちょうど尿試験紙のようなストリップで内視鏡内の内腔の還流水でこれらの 3 種類の物質を検出できる。
