中心ライン関連血流感染症の可能性がある血液内科患者における早期ライン抜去およびライン留置継続成功の予測因子:後向きコホート研究★★

背景

造血器悪性腫瘍を有する患者には、中心ライン関連血流感染症(CLABSI)およびその有害な転帰のリスクがある。ライン留置継続方針が適している患者はまだ明確に定義されていない。

目的

血液内科患者に CLABSI の可能性がある場合のライン留置継続の失敗を予測しうる因子を評価すること、およびラインの抜去および留置継続に関する実態を記述すること。

方法

中心ライン留置中に BSI が発現した血液内科入院患者を対象として、単一施設後向きコホート研究を実施した。ライン留置継続とは、BSI 発現後 72 時間以上そのままラインを留置することと定義した。主要評価項目は、BSI 後 72 時間を超えてからのライン抜去、または同じ微生物による 3 か月以内の BSI 再発と定義した、ライン留置継続失敗とした。多変量解析を用いて、ライン留置継続失敗、早期のライン抜去(72 時間未満)および米国感染症学会(Infectious Diseases Society of America)中心ライン関連BSI(CRBSI)(研究定義 CRBSI)の予測因子を評価した。

結果

2018 年 1 月 1 日から2022年 12 月 31 日に BSI エピソード 288 件が認められ、そのうち209 例に72 時間を超えるライン留置継続があり、そのうち 52例(24.9%)でライン留置継続失敗が発生した。多変量解析において、培養陽性の延長がライン留置継続失敗の正の予測因子であり、BSI 感染源がライン以外であることがライン留置継続成功と関連していた。79 例(27.4%)は早期にラインが抜去され、集中治療室入室、培養陽性の延長および研究定義 CRBSI に合致することなどの因子と関連していた。BSI エピソードのうち 27 件(9.4%)のみが研究定義 CRBSI に合致していた。

結論

ライン抜去前に、別の感染源について詳細に評価すること、および反復血液培養などの非侵襲的診断法を使用することにより、適切なライン留置継続を増やすことができる可能性がある。

監訳者コメント

CRBSI の可能性がある場合、CVカテーテル抜去が基本とされている。本研究では、CRBSI以外の感染症のフォーカス評価、血液培養の再検査、DTP(differential time to positivity)法による判定を行うことで、CRBSI の可能性が低い場合には、CVカテーテル留置を継続しうることが示唆された。
