背景

バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium)(VREfm)は重要な院内病原体である。配列タイプ 80(ST80)vanA を有する VREfm はアイルランドの病院で優勢であるが、その伝播については十分理解されていない。

目的

アイルランドの病院 1 施設(H1)の 2 つの病棟において、優勢な複合体タイプ(CT)VREfm の伝播および持続について、全ゲノムシークエンシング(WGS)を用いて検討し、それらの病院内および病院間の拡散について評価した。

方法

直腸スクリーニング(N330、2019 年 9 月から 2022 年 12 月まで)および環境サンプリング(N = 48、2022 年 11 月から 2022 年 12 月まで)における E. faecium を調べた。分離株の類縁性を、コアゲノム複数部位塩基配列タイピング(cgMLST)およびコアゲノム一塩基多型(cgSNP)解析により評価した。強く疑われる感染伝播経路は SeqTrack(https://graphsnp.fordelab.com/graphsnp)を用いて、cgSNP データおよび回収地点に基づいて特定した。H1 を含めたアイルランドの病院 7 施設から得られ、特性が明らかにされた E. faeciumN = 908、2017 年 6 月から 2022 年 7 月まで)についても検討した。

結果

従来の MLST により、分離株は 9 つの ST(ST80、82%)に分類された。cgMLST により、類縁の分離株(アレル差が 20 アレル以下)に 3 つの主要な ST80 CT(CT2933、CT2932 および CT1916)(分離株の 55%)が同定された。cgSNP 解析により、これらの CT 株に複数の密接に関係するゲノムクラスター(cgSNP 10 以下)が識別された。Parisimonious network construction により、入院 30 日以内の患者間における疫学的根拠とともに病棟内および病棟間に強く疑われる感染伝播 55 件が特定され、これには 7 つのゲノムクラスター中の分離株 73 株(cgSNP 10 以下)が含まれた。より長期にわたり、明らかな疫学的根拠の認められない、その他の強く疑われる感染伝播が多数特定され、このことから持続と、特定されていないリザーバが拡散に寄与していることが示唆された。病院 7 施設では、3 つの CT が E. faeciumN = 1,286)において優勢であり、このことから病院間の拡散には疫学的関係が不明であることが強調される。

結論

本研究により、長期の病院内および病院間において 3 つの主要な CT ST80 VREfm 系統が優勢であること、広範な伝播ならびに持続が明らかになり、これらから特定されていないリザーバがあることが示唆された。

監訳者コメント

近年次世代シーケンサーの進化により以前よりも低予算で高速にゲノム解析が容易となっている。この手法を利用した cgMLST は、従来の MLST などの分子疫学的分析よりもより多くのゲノム配列が解析できる結果、菌株間の識別能力が上がり、本論文のように VRE の伝播が病院内のみならず施設を越えた拡がりであることが証明されるとともに、疫学的関連性を証明できない事例が確認されることとなり、これらの関連性を理解するためには、より詳細な疫学調査が必要となる。
