Hospital-based antimicrobial stewardship in Denmark, Greenland and the Faroe Islands—current landscape and barriers

目的

デンマーク、フェロー諸島、グリーンランドにおける正式化された集学的な病院主体の抗菌薬適正使用支援構造の現時点での組織と実施について記述すること。

方法

構造化された電子質問票を、臨床微生物学（N ＝ 207）および感染症（N ＝ 260）の研修生および専門家全員と、感染症が専門の臨床薬剤師（N ＝ 20）および小児科医（N ＝ 10）に送った。調査は、病院における抗菌薬適正使用支援に関する国際コンセンサスチェックリストに基づき、デンマークの状況に合わせた、多肢選択式の質問、評価尺度を使った質問、自由回答式の質問 30 個から成っていた。

結果

全体で、20 の病院を代表する 145 名から回答を得た。9 つの病院（45％）が、正式な抗菌薬適正使用支援戦略を報告し、8 つ（40％）が、正式な組織化された集学的な構造と、集学的な抗菌薬適正使用支援チームを報告し、6 つ（30％）が、抗菌薬適正使用支援チームのリーダーとして専門家が指名されていることを報告した。大多数の病院で、最新のガイドラインにアクセスできることが報告され（80％）、処方された抗生物質の量が定期的に監視され（70％）、報告されていた（65％）。抗菌薬適正使用支援専用の十分な予算が維持されていることを報告したのは、1 つの病院のみ（5％）で、3 つの病院（15％）は、特定の病原体／臨床状態に対する治療の経過について監査していて、4 つの病院（20％）は、抗菌薬適正使用支援における役割、協力の方法、責任について明確に定義した文書を持っていた。回答者全員のうち、合計で 42％が抗菌薬適正使用支援の正式な訓練を受けていた。主な障壁は、財源の不足（52％）、病院の経営者側から権限が与えられていないこと（30％）、抗菌薬適正使用支援が優先事項ではないこと（18％）であった。

結論

病院主体の集学的な抗菌薬適正使用支援にとって重要な中核的な要素は、デンマークの病院において強化可能である。病院の経営者側から資金と明確な権限を与えられること、優先事項となること、集学的な抗菌薬適正使用支援構造が実施されることが、特定されたギャップを狭めるのに役立つ可能性がある。

アンケート調査によるデンマーク王国（グリーンランド、フェロー諸島を含む）の抗菌薬適正使用支援の現状と課題の報告。それぞれの国の事情があると思うが、資金や病院経営陣の理解などの課題は日本と大きく変わらなそうである。
