Adverse events associated with patient isolation: a systematic literature review and meta-analysis

背景

多剤耐性病原体（MDRO）の伝播を予防するため、医療機関において患者隔離が広く推奨され実施されている。しかし、そのリスク・ベネフィット比については議論中である。

目的

新たに発表された研究に基づいて、患者隔離に関連する身体的・精神的な有害事象が存在するかどうか評価すること。

方法

Preferred Reporting Items for Systematic reviews and Meta-Analyses（PRISMA）ガイドラインに従って、文献のシステマティックレビューとメタアナリシスを実施した。PubMed で 2009 年 5 月 1 日から 2020 年 1 月 31 日までの研究を、系統的に検索した。研究課題の概念は、「有害事象」、「患者隔離またはコホーティング」、「多剤耐性病原体保菌患者または感染患者」と定義した。3 名の評価者が独立して研究をスクリーニングし、データを抽出した。すべての統計解析は Stata ソフトウェアおよび R ソフトウェアを用いて実施した。

結果

15,921 報の論文のタイトルおよび 196 報の抄録をスクリーニングし、50 報の研究の全文をレビューした後、19 報の研究を対象とした。研究を、定性的研究（4 報）、メタアナリシスに適した観察研究（7 報）、その他の観察研究（8 報）の 3 グループに分類した。メタアナリシスによって、診療に関係する有害事象も患者隔離に関連する患者の経験に関係する有害事象も認められないことが示された。

結論

我々の結果を裏付けるためには、対照群および標準化された選択基準を含む正しい方法論を用いたより多くの研究が実施される必要がある。

監訳者コメント：

これまで耐性菌の検出された患者等に対して実施されていた「接触隔離予防策」については一定の効果は認められるものの、提供する医療の質や隔離された患者への隔離による精神的な弊害については研究のモデルとしての対照の設定がないため、十分に検討できていない状況がある。本研究でのメタアナリシスによっても十分な結果を見いだすことはできなかった。今後、ますます耐性菌の問題は社会問題化するため、解決できていないあるいは不明な点についてはさらなる研究が必要である。
