感染制御の医療従事者への負担：スコーピングレビュー★★

緒言
病院感染（HAI）は患者に重大なリスクをもたらし、感染予防・制御策（IPC）の主要な焦点となる。IPC のコンプライアンスが総じて低い理由の 1 つは、十分に認識されていないが、IPC が医療従事者にとって負担になることである。
目的
IPC の医療従事者への負担を明らかにした。
方法
PubMed および CINAHL において、2000 年以降に英語で発表された、IPC のコンプライアンスおよびコンプライアンスに伴う負担に関する研究を検索した。最初の結果として得られた 1,018 報をスクリーニング後、25 報の論文を最終レビューに組み入れた。
結果
皮膚科合併症、頭痛、感覚症状、時間的制約などの負担に関するエビデンスが見出された。IPC のコンプライアンスを評価するために作成されたツールには限界があり、コンプライアンスの負担が評価されることはまれである。強い安全文化から前向きなコンプライアンスが予測された一方、IPC の基礎にある根拠に関する知識には、コンプライアンスとの非線形関係が認められた。
結論
今後の研究によって、IPC に関連する負担と、それらの負担を最小限に抑え、コンプライアンスの向上を達成するための方法を明らかにすべきである。
監訳者コメント：
感染対策における医療従事者側の負担は遵守率低下につながるため、その状況把握は重要である。PPE 着用時の不快感は約 80％、皮膚に関するトラブルでは 85％の看護師が経験しており、なかでも皮膚乾燥による皮膚のトラブルは最も頻度の高いもので、医療従事者の約 50％との報告がある。COVID-19 の大流行は、マスク着用による接触性皮膚炎や湿気による湿疹やニキビ、マスクの圧迫による皮膚障害などを顕在化させた。また PPE 着用は視覚や聴覚の低下を伴い、結果的に意思疎通の障害や PPE を外した後の頭痛などを引き起こす結果となっている。遵守率を低下させる原因としてはこれまで我々は皮膚トラブル以外あまり注目してこなかったが、COVID-19 のパンデミックにより医療従事者への多様な負担が明らかとなってきており、これらへの対処を含めた感染対策が必要である。
