背景

黄色ブドウ球菌(Staphylococcus aureus)シークエンス型 5(ST5)は世界的な脅威になりつつある。

目的

麻酔作業区域における ST5 の伝播の疫学的特徴を明らかにすること。

方法

この後向きコホート研究では、麻酔作業区域におけるリザーバが関与し、予防投与の抗菌薬に耐性で、伝播の見られた黄色ブドウ球菌分離株を解析した。全ゲノム解析を使い、ST5 の伝播の疫学的特徴を、感染源となったリザーバ、伝播の場所、侵入門戸、伝播の方法ごとに明らかにした。手術部位感染症(SSI)の発生に関して、全患者を少なくとも 30 日間追跡調査した。

結果

分離株の 41%(18/44、95%信頼区間:28 ~ 56%)が ST5 であった。感染源となったリザーバーが医療提供者の手であったのは、伝播の見られた ST5 で 28%(5/18)であったが、その他の ST では 4%(1/26)であった。伝播の場所が医療提供者の手であったのは、ST5 では 28%(5/18)であったが、その他の ST では 7%(2/26)であった。活栓のコンタミネーションが ST5 の分離株の 8%(1/13)で起こり、その他の ST では 12%(3/25)であった。手術日が異なる症例間で起こった伝播イベントの 63%に ST5 が関わっていた。ST5 は、耐性の特徴を持っている可能性が高く(中央値[四分位範囲]は、ST 5が 3[2 ~ 3]、その他の ST が 2(1 ~ 2)、P < 0.001)、セファゾリン、ピペラシリン–タゾバクタム、および/またはシプロフロキサシンに対する耐性が強かった(ST5 が 3[2~3]、その他の ST が 2(1 ~ 3)、P = 0.02)。ST5 は、6 件の SSI のうち 3 件と関連していた。

結論

ST5 は、麻酔作業区域において、予防投与の抗菌薬に耐性で、伝播の見られた分離株のなかによく見られた。伝播には医療提供者の手が関与し、1 人の患者から、後日、別の患者へ伝播する。ST5 は、術中のその他のメチシリン耐性黄色ブドウ球菌よりも、多くの薬剤耐性因子、および試験管内での感受性の減少と関連していた。
