背景

関節全置換術を受ける患者においては、術後の関節感染を予防するために、入院前の鼻腔内メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)スクリーニングが広く採用されている。しかし、スクリーニングの費用対効果と臨床的有用性は十分に評価されていない。

目的

スクリーニング導入前と導入後の MRSA 感染率、関連する費用、当施設でのスクリーニングの費用を評価した。

方法

本研究は、米国ニューヨーク州の一医療機関で 2005 年から 2016 年の間に関節全置換術を受けた患者を対象とした、後向きコホート研究であった。2011 年の MRSA スクリーニングプロトコール導入より前に手術を受けた患者を「スクリーニングなし」群、それ以降に手術を受けた患者を「スクリーニング」群として分類した。MRSA 関節感染数、各感染の費用、術前スクリーニングに伴う費用を記録した。Fisher の正確確率検定と費用比較分析を実施した。

結果

スクリーニングなし群では 7 年間に患者 6,088例において MRSA 感染 4 件が発生した一方、スクリーニング群では 5 年間に 5,177 例において 2 件が発生した。Fisher の正確確率検定により、スクリーニングと MRSA 感染率の間に有意な関連は認められなかった(P = 0.694)。術後の MRSA 関節感染治療の費用は、患者あたり 40,919.13 米ドルであった一方、鼻腔内スクリーニングの年間費用は 103,999.97 米ドルであった。

結論

当施設では、MRSA スクリーニングは感染率にほとんど影響を及ぼさず、費用の増加を招き、スクリーニングの費用に見合うためには年間 2.5 件の MRSA 感染が必要であった。したがって、スクリーニングプロトコールは、関節全置換術を受ける平均的な患者ではなく高リスク集団に最適であると考えられる。MRSA スクリーニングプログラムを実施している他の施設でも、同様の臨床的有用性および費用対効果の分析を行うことを推奨したい。

監訳者コメント


関節全置換手術における鼻腔 MRSA スクリーニングの有用性を後方視的に検討したものである。手術対象患者を、一律にスクリーニングをするのではなく、CA-MRSA と HA-MRSA での差はあるのか?また、ICU 入室、抗菌薬曝露歴、手術歴、血液透析、慢性的な創傷(褥瘡や創部)などの MRSA 保菌リスクの高い患者に対してスクリーニングをした場合での費用対効果を検討する必要があろう。
