背景

医療関連感染症(HAI)と抗菌薬耐性(AMR)は、深刻な健康上の課題である。点有病率調査(PPS)は、HAI と AMR のモニタリングに有用なツールである。

目的

急性期ケア病院における HAI の有病率、抗菌薬使用量、ならびに関連因子を扱った ECDC PPS 2022 の結果について述べること。

方法

本調査は、欧州疾病予防管理センター(ECDC)が提示したプロトコールに従って、2022 年 11 月に 14 施設の病院で実施された。HAI と抗菌薬に独立して関連する因子を評価するために、クラスター変数として地理的地域/病院の種類を用いてマルチレベル・ロジスティック回帰を実施した。

結果

HAI の点有病率は 7.43%であった。長期入院患者は、15 歳から 44 歳の患者、急性致死性疾病患者、挿管患者、1 つまたは 2 つのデバイスを有する患者と同様に HAI を有する傾向が強かった。抗菌薬使用率は 47.30%であった。男性、McCabe スコアが不明、低侵襲手術/国家的医療安全ネットワーク(NHSN)が定義した手術以外の手術、HAI 患者、擦式アルコール製剤の使用量がより多い病院、感染予防・制御(IPC)担当者の人数がより多い病院、高齢者病棟、300 床 から 600 床の病院では、抗菌薬療法を実施している傾向が強かった。

結論

本 PPS によって、HAI の有病率、抗菌薬使用量および関連する変数に関する有益な情報がもたらされた。HAI の有病率の高さは、感染制御策の改善の必要性を浮き彫りにしている。

監訳者コメント


欧州の中でも特に薬剤耐性菌による疾病負荷が大きいイタリアからの報告。点有病率調査(PPS)は費用対効果が高く、信頼性が高い方法で、本調査では PPS を用いて、医療関連感染症(HAI)発生率と抗菌薬の使用量を推定している。擦式アルコール製剤の使用量がより多い病院、感染予防・制御(IPC)担当者の人数がより多い病院で抗菌薬療法を実施している傾向が強い点については説明が難しいが、これらの要素が原因というよりは結果と捉えるべきだろう。
