外来でのスクリーニングと除菌による術前の黄色ブドウ球菌定着の排除:患者の視点から見た外来除菌の実効性★

背景
病院における黄色ブドウ球菌(Staphylococcus aureus)症例の 90%は市中感染であるが、除菌戦略は主に入院患者を対象としている。Preoperative Detection and Decolonisation for the Prevention of Staphylococcus aureus Complications in Elective Patients(STAUfrei)プロジェクトは、外来でのスクリーニングと入院前の除菌による分野横断的アプローチについて探索した。本研究では、実施可能性、患者アドヒアランス、およびそれらの除菌の成功との関連を評価した。
方法
ドイツ南西部の病院で待機的手術を予定された患者8,054例(>18歳)のうち、2019 年から 2022 年までの登録時に黄色ブドウ球菌陽性であったすべての介入患者(3,390例)にサブスタディへの参加を依頼した。質問票および除菌日誌を用いてデータを収集した。保菌患者は1日3回投与による 5 日間のレジメンを完了した。質問票のデータを記述的に解析し、アドヒアランスと入院時の保菌との関連をロジスティック回帰により評価した。
結果
計 2,147 例の患者が調査に参加した。除菌日誌に 1 回以上入力し、ベースライン時に黄色ブドウ球菌陽性であった患者のみを解析対象とした(N = 588)。除菌対策に対する患者受容度は高く、86.9%は実施可能性が「良好~非常に良好」であると報告し、大半が期間(87.4%)と毎日の労力(78.7%)の両方を許容可能と考えていた。4 分の 1 で対策に対するアドヒアランスが 80%を超えていたが、完全なアドヒアランスが認められたのは 6.6%のみであった。昼間のアドヒアランスが最も低かった。アドヒアランスの高さは保菌率の低さと関連していたが、統計学的に有意ではなかった。
結論
質問票のデータから、高い実施可能性と自宅での除菌に対するアドヒアランスが示された。アドヒアランスの高さは、入院時の保菌率の低さと関連していた。外来での効果的な対策の最適な構成を定義するためには、さらなる研究が必要である。
