背景

クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)の臨床的および経済的負担が増加しつつあるにもかかわらず、アジア太平洋地域の集中治療室(ICU)における CDI に関するデータは少ない。

方法

本後向き研究では、2017 年 1 月から 2021 年 5 月にかけて韓国の病院 3 施設の ICU で CDI に対する治療を受けた患者 191 例の解析を行った。後退型段階的線形多重ロジスティック回帰分析を用いて、治療効果および死亡率に影響を及ぼす因子を同定した。

結果

患者58 例(30.4%)が免疫不全と考えられた。Charlson 併存疾患指数スコアの平均は 5.65 ± 2.39(10 年生存率 21%)、APACHE II スコアの平均は 20.86 ± 7.78(死亡率 40%)、ATLAS スコアの平均は 5.45 ± 1.59(治癒率 75%)、ならびに SOFA スコアの平均は 7.97 ± 4.03(死亡率 21.5%)であった。CDI 症例中 58 例(30.4%)は重症、40 例(20.9%)は劇症型であった。経口バンコマイシンまたは経口メトロニダゾールは、最も多く用いられていた第一選択治療であった(N = 57、32.6%)。10 日治療効果率は 59.7%、8 週全死亡率は 41.4%であった。劇症型 CDI(OR 0.230、95%信頼区間[CI]0.085 ~ 0.623)および SOFA スコアの 1 単位増加(OR 0.848、95%CI 0.759 ~ 0.947)が治療失敗と関連していた。APACHE II スコア(OR 0.355、95% CI 0.143 ~ 0.880)および SOFA スコア(OR 0.164、95% CI 0.061 ~ 0.441)の高値が死亡率上昇と関連していた。

結論

ICU における高リスク患者は CDI の死亡率が高く、治癒率が低かった。より正確な予測スコア判定法およびより良好な臨床転帰を得るためには、さらなる研究が必要である。

監訳者コメント


ICU 入室患者に発生する CDI は予後が悪く、再発率も高いことが欧米と同様韓国においてもわかった。本研究では血液疾患や悪性腫瘍などの免疫不全患者が多いことも予後の悪さを反映している。APACHE II スコアと SOFA スコアの高得点と生存率の低下との関連性が見いだされた。治癒率や再発率が高いことから予後改善には、バンコマイシンやメトロニダゾールの一般的治療以外の選択として、フィダキソマイシン、バンコマイシンのパルス・漸減療法、ベズロトクスマブ、糞便移植なども考慮する必要があるだろう。
