Investigation of in-line filter replacement intervals for infusion

背景

末梢および中心静脈カテーテルのインラインフィルターは、細菌細胞を機械的に除去するために用いられる。最近の研究では、輸液セットの使用期間を 7 日間まで延長できることが示された。インラインフィルターを交換する間隔についてのエビデンスは存在しない。

目的

7 日間にわたり継続使用されたインラインフィルターについて試験を行い、その細菌除去能力を検討し、その流量を評価すること。

方法

3 種類のインラインフィルターを高カロリー輸液用エルネオパ NF2 号輸液に装着し、その中に表皮ブドウ球菌（Staphylococcus epidermidis）ATCC12228 または大腸菌（Escherichia coli） ATCC25922 を接種した。これらの実験結果を、対照輸液と比較した。輸液は、流量 40 mL/時で点滴し、7 日間にわたり 24 時間間隔で交換した。点滴開始から 24 時間後にサンプルを採取した。

結果

表皮ブドウ球菌は、1 日目から 6 日目の間には液滴中に検出されなかったが、7 日目にはインラインフィルター 1 および 2 の液滴中に、6 ～ 10 コロニー形成単位（CFU）/mL が検出された。大腸菌は、7 日間の連続使用後にいずれのフィルターにも検出されなかった。流量の 40 mL/時への低下は、表皮ブドウ球菌の試験では7 日目にフィルター 3 で、また大腸菌の試験では 4 日目にインラインフィルター 2 で、3 日目にインラインフィルター 3 で認められた。

結論

本研究では、高接種条件下で試験を行った 3 種類のインラインフィルターにおいて、細菌除去および流量に差があることが明らかにされた。インラインフィルターは連続して最長 6 日間使用できること、また 48 時間の連続使用後は流量の低下に慎重に注意すべきであることが示唆される。
