背景

黄色ブドウ球菌(Staphylococcus aureus)は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)とメチシリン感受性黄色ブドウ球菌(meticillin-sensitive Staphylococcus aureus;MSSA)のいずれも、新生児における感染症の重要な原因である。感染制御策の実施によって、MSSA 感染症の発生率は MRSA 感染症と同程度にまで低下したわけではない。

 

目的

新生児集中治療室(NICU)における MSSA の伝播経路について、遺伝子解析を用いて検討すること。

 

方法

2018 年 10 月 1 日から 2019 年 3 月 31 日に当院の NICU において、入院時およびその後は毎月 1 回、新生児患児、その親、および医療従事者に対してスワブ検査を実施した。全ゲノムシークエンシングを実施して MSSA 株を検出した。Multi-locus sequence typing(MLST)および一塩基多型(SNP)解析を用いて、菌株の同定およびそれらの近縁性の解明を行った。

 

結果

MSSA 陽性患児は 16 例であった。MSSA 陽性患児 4 例では、医療従事者の分離株と同じ系統群に属することが認められた。患児 1 例では、親と同じ分離株が認められた。MSSA 陽性の双子の新生児には、同じ分離株が認められた。患児 10 例は散発株であり、検査を受けた医療従事者 97 名中 32 名は MSSA 陽性であった。

 

結論

本研究の結果から、NICU における MSSA の伝播経路には、病院環境における MSSA を介するものに加えて、医療従事者からの水平伝播があることが考えられる。NICU における MSSA を対象とした感染制御には、アルコールによる手指衛生とともに、徹底した環境管理と親に対する教育が重要である。

監訳者コメント

順天堂大学からの報告。新生児領域では MRSA と同様に MSSA も制御すべき感染症であり、NICU における MSSA の伝播について分子疫学的に調査した報告。カンガルーケアなど接触期間の多い親からの伝播が多いことが予想されたが、今回 MLST および SNP 解析で解析した結果、親からの伝播が疑われた事例は 1 例しかなかった点は興味深い。今後、多施設での解析を期待したい。
