A型肝炎ウイルスの無生物表面での安定性と不活化★

背景
A 型肝炎ウイルス(HAV)は、ウイルス性肝炎の主因の 1 つであり、毎年 1.59 億件の急性感染が起こると推定されている。伝播の主要経路は、汚染された飲み水や食品を介しての糞口経路である。HAV の表面での安定性や、表面消毒薬に対する感受性に関して入手できるデータは限られている。有効な消毒薬を使うことは、HAV の伝播の予防にかなりの影響を与えられる可能性がある。
目的
HAV の安定性および表面消毒薬に対する感受性を、確立しているキャリアアッセイに基づいて評価すること。
方法
ステンレス鋼の円盤上での HAV の安定性を、60 日間に亘って評価し、市販されている様々な表面消毒薬に対する感受性を評価した。鋼製円盤であるキャリアに HAV の粒子を播種し、それぞれの期間に亘ってインキュベートした。さらに、消毒剤のいくつかのグループに対する HAV の抵抗性を、製造業者のガイドラインに従い、様々な濃度と曝露時間で検査した。
結果
HAV は、ステンレス鋼の円盤から、最長で 40 日間回収可能であり、半減期は 18.63 日と推定された。いくつかの表面消毒薬の評価では、アルデヒドに基づく製品を除くすべての製品が、HAV を十分に不活化できなかった。
結論
全体として、HAV は、試験した広い範囲の表面消毒薬に対して、高い抵抗性を示した。評価した 9 つの表面消毒薬のうち、アルデヒドに基づく2つの製品のみが、検出限界未満までウイルス価を減少させた。これらの結果は、HAV の伝播を減少させることを目的としたエビデンスに基づく衛生ガイドラインの推奨に対して、強い影響を与える。
監訳者コメント :
A型肝炎ウイルスはエンベロープを持たない小型球形ウイルスであり、アルコールや界面活性剤などの消毒薬に抵抗性を示す。本研究でも消毒薬への強い抵抗性を裏付けている。
