背景
カルバペネム耐性アシネトバクター・バウマニー(Acinetobacter baumannii)(CRAB)は、priority 1 の最重要な病原体の 1 つであり、治療の選択肢が限られていて、集中治療室(ICU)の患者における死亡率が高い。イタリアが高いレベルの流行を報告している一方で、フランスは低い罹患率を維持している。国境を挟んだ違いを理解することは、感染予防・制御(IPC)および抗菌薬適正使用支援戦略に情報を提供するために不可欠である。
目的
感染リスクと関連する因子を特定する目的で、フランスとイタリアの国境を挟んだ地域にある ICU において、CRAB 感染の発生率の傾向を調べ、ICU の構造と組織、IPC、適正使用支援の実践について評価した。
方法
国境を挟んだ 4 つの地域(Piemonte、Valle d’Aosta、Auvergne-Rhône-Alpes、Provence-Alpes-Côte d’Azur)の ICU を対象とした多施設共同観察研究を実施した。ICU で感染した血流感染症(BSI)、中心ライン関連 BSI、肺炎、人工呼吸器関連肺炎で CRAB の単離株があったものについてのデータをGiViTI および Réa-Rézo サーベイランスシステムから抽出した(2019 年から 2022年)。構造化された調査で、ICU の特徴、IPC、適正使用支援の実践について調べた。CRAB 感染のエピソードと関連する因子を特定するのにロジスティック回帰分析を実施した。
結果
全体で、25 の ICU がデータの収集に参加した。ICU への入室 24,822 件のうち、カルバペネム耐性であったA. baumannii 単離株の割合は、フランスが 18.75%(95%信頼区間[CI]、7.97 ~ 34.98)、イタリアが 83.56%(95%CI、80.1 ~ 86.63)であった。IPC および抗菌薬適正使用支援への取り組み方が違うことがはっきりした。ICU の構造のデザイン、職員の配置の柔軟性、IPC および適正使用支援のしぶとい実践が感染リスクの低下と関連していた。
結論
CRAB 感染の発生率は、イタリアの方がフランスよりもかなり高かった。国境を越えて広がることを防ぐには、サーベイランスを国境を挟んで一致させることと、予防戦略をうまく調整することが極めて重要である。

イタリアとフランスの ICU における CRAB 発生率の差と、その背景にある感染予防と抗菌薬適正使用の違いが明らかになった。国境を越えたサーベイランス体制の連携強化の必要性を示した。
