背景

カルバペネム耐性腸内細菌目細菌(CRE)は、集中治療室(ICU)の中において、深刻なそして増大している脅威である。それは、極めて高い死亡率、入院期間の延長、およびかなりの額の医療費と関連している。

目的

この研究の目的は、ICU滞在中に発生する CRE感染および/または保菌を予測するノモグラムを開発し妥当性を検証することであった。

方法

この積極的サーベイランスを行う前向き多施設共同コホート研究には、2,149 例の ICU 患者が組み入れられ、臨床感染についてのシステマティックなスクリーニングと直腸スワブの採取を含む網羅的なモニタリングの対象とされた。ICU における CRE 感染および/または保菌の予測因子の選択を、予測因子の最適なプールを得るためのラッソ(least absolute shrinkage and selection operator、LASSO)回帰および赤池情報量基準を使って実施した。その結果に基づくノモグラムを、トレーニングコホート(6 つの ICU)で構築し、それとは別のコホート(4 つの ICU)で外部検証した。評価には、識別能、較正、および臨床的有用性を含めた。

結果

ICU 滞在中のCRE 感染/保菌率は 14.5%であった。CRE 感染/保菌という転帰を予測するための主要モデルは、11 個の独立した予測因子を組み入れたものであったが、強固で汎用性の高い結果を示した。達成された曲線下面積(AUC)は、トレーニングセットで 0.757、外部検証で 0.764 であった。このモデルは、十分に較正されていて(較正傾き、0.874)、規定されたリスク閾値範囲が 12% ~ 55%以内である正味の臨床的利益をもたらした。CRE 感染のみや保菌のみを予測する特異的なモデルも良好な識別能を示し、外部検証の AUC は、それぞれ 0.794 と 0.730 であった。

結論

この研究は、ICU で CRE に感染するリスクの高い患者を特定するノモグラムを提示している。これによって、標的を絞ったスクリーニングや予防策を容易にするための個別化されたリスク評価が可能になる。

監訳者コメント:
中国の 9 病院・10 ICU、2,149 例を前向きに調査し、ICU 入室後のCRE 感染・保菌リスクを予測するモデルを開発した。CRE 感染・保菌の発生率は 14.5%で、11 の予測因子を用いたモデルは外部検証でも良好な予測性能を示した。無症候性のCRE保菌者は院内伝播のリザーバーとなるため、高リスク患者を予測して直腸スクリーニングや感染対策を重点化する「リスクベースのサーベイランス」への応用が期待される。「CRE が出てから対応する」から「CRE を獲得しそうな患者を予測して先回りする」への発想転換が、本研究の感染対策上興味深い点である。
