ウクライナの 2014 から 2023 年における戦争に関係した外傷の創傷感染の細菌種およびそれらの抗菌薬耐性の特徴の時間的な変化★

背景
この研究では、ウクライナにおいて、戦争での外傷に関連した抗菌薬耐性についてのサーベイランスを続けている。
目的
種の組成、抗菌薬耐性プロファイル、新たな耐性遺伝子の出現を、2014 から 2020 年と 2022 から 2023 年の間で、比較すること。
方法
これは、ウクライナの病院における後向き多施設共同微生物学的調査であった。抗菌薬感受性、全ゲノムシークエンシング、複数部位塩基配列タイピングを、2022 から 2023 年の間に、125 人の死傷者から得られた 154 個の微生物について実施した。
結果
データから、グラム陰性菌、特に、アシネトバクター・バウマニー(Acinetobacter baumannii)(35.7%)、緑膿菌(Pseudomonas aeruginosa)(14.9%)、肺炎桿菌(Klebsiella pneumoniae)(20.7%)が多いことが明らかになった。高いレベルのカルバペネム耐性が、A. baumannii(メロペネム、72.2%[39 / 54、95%信頼区間{CI}58.4 ~ 83.5]、イミペネム、66.7%[36 / 54、95%CI、52.5 ~ 78.9])、肺炎桿菌(メロペネム、90.6%[29 / 32、95%CI、75.0 ~ 98.0]、イミペネム、81.2%[26 / 32、95%CI、63.6 ~ 92.8])、緑膿菌(メロペネム、47.8%[11 / 23、95%CI、26.8 ~ 69.4]、イミペネム、60.8%[14 / 23、95%CI、38.5 ~ 80.3])株の中に観察された。A. baumannii の配列型(ST)-78 と ST-400 が 2014 から 2020 年までの間広まっていたが、2022 から 2023 年では、ST-1077 の 5 つの株が新たに検出された。緑膿菌株は 16 個の ST という多様性を示し、ST-773 の頻度が増加し、新たな ST が出現したけれども、カルバペネマーゼ遺伝子は欠けていた。肺炎桿菌は、遺伝的多様性が徐々に増加し、2014 から 2020 年までは 3 つの ST が存在し、2022 から 2023 年には、blaNDM-1、blaOXA-48、blaKPC2 を保有するものを含む 6 つの新たな ST が現れた。
結論
世界中に広がるリスクが高いことと関連している ST の多剤耐性分離株の存在率が増加している。
監訳者コメント :
ウクライナにおける戦争関連外傷の限られた状況であるが、アシネトバクター・バウマニー、肺炎桿菌、緑膿菌といったグラム陰性桿菌のカルバペネム耐性率の高さに愕然とする。日本ではこれらグラム陰性桿菌のカルバペネム耐性が低い傾向があるが、AMR 蔓延国と同じ轍を踏まないように AMR 対策の重要性を理解できる報告である。
