背景
クロストリジウム・ディフィシル(Clostridium difficile)感染症(CDI)の医療費への影響は、それに伴う入院患者のケアの超過費用のために甚大なものとなる。しかし、再発の影響に焦点を当てた研究はこれまでほとんどない。
目的
本研究の目的は、フランスの急性期病院の CDI による病院費用、および再発に起因する費用の割合を推計することである。
方法
12 の大規模急性期病院のサンプルに基づき、2011 年に後向き研究を実施した。CDI の費用を病院の視点と公的保険の視点の両方から推計した。CDI による追加費用の推計にあたっては、それぞれの入院ごとに、全国 DRG(diagnosis-related group;診断群)データベースから抽出した、DRG、年齢、および性別をマッチさせた CDI がない対照との比較を行った。CDI が主診断である場合は入院の総費用を使用した。
結果
微生物学的に確認された CDI 症例 1,067 例を特定し、重複のない 906 例が合計 979 件の入院をしていた。これらの入院のうち 118 件(12%)は再発例であることが確認され(複数回の再発もあり)、その 51.7%は初発エピソードと同一の入院で発生していた。それら(初発 + 再発)の平均入院期間は 63.8 日、初発エピソードのみの場合は 25.1 日であった。CDI による入院 1 件あたりの平均超過費用推計値は 9,575 ユーロ(中央値 7,514 ユーロ)であった。公立急性期病院における全国レベルでの CDI による超過費用は 1 億 6310 万ユーロと推計され、このうち 12.5%は再発に起因するものであった。
結論
フランスでは CDI による経済的負担は甚大であり、医療制度に直接的な影響を及ぼしている。

近年、CDI は EU 諸国では強毒株 BI/NAP/01 が主流であり、フランスでも発生率は患者 10,000 人あたり 2.2(2009 年)から 3.6(2012 年)と増加している。CDI 発生にかかわる費用は在院日数の増加、再入院、検査コスト、治療にかかる費用として考えられる。今回は、CDI の発生とその再発による超過費用が莫大であることが証明され、感染予防対策と再発予防の重要性を再確認することとなった。
