MRSA 保菌 を標的とした除菌はその後の MRSA 感染リスクを低減する:準実験的研究★

背景
病院全体においてメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)を標的とした除菌プログラムが、その後の MRSA 感染および/または菌血症のリスクに及ぼす影響を検討した。
方法
本研究は前後比較研究で、香港の大学病院付属病院 1 施設で実施した。入院患者を対象に、ムピロシン鼻腔内投与およびクロルヘキシジン浴またはクロルヘキシジン浸漬タオルを用いた MRSA 除菌を、2024 年 8 月 27 日以降にMRSA を有する全成人患者に実施した。2024 年 8 月 27 日から 2025 年 8 月 20 日の間に除菌を受けた MRSA 患者を介入群とした。2023 年 8 月 4 日から 2024 年 8 月 10 日の間に除菌を受けなかった MRSA 患者を対照群とした。全患者に対して、2025 年 11 月 20 日まで追跡調査を行った。
以下の 2 つの転帰を調べた:1.すべての臨床試料において次に MRSA が検出されるまでの期間、および 2.血液培養において次に MRSA が検出されるまでの期間。
多変量 Cox 比例ハザード回帰を用いて、この介入が初回 MRSA 感染陽性までの期間、ならびに次の MRSA 菌血症までの期間に及ぼす影響について解析した。
結果
介入群は 932 例、対照群は 996 例であった。患者の年齢中央値は 79 歳、男女比は 1.56:1 であった。介入群では、その後の MRSA 感染リスクが低かった(補正ハザード比[aHR]0.77、P = 0.015)。介入群では、その後の MRSA 菌血症に減少の傾向が認められた(aHR = 0.52、P = 0.054)。
結論
病院全体における MRSA保菌者を標的とした除菌は、除菌後の MRSA 感染リスクの低下と関連した。
監訳者コメント :
著者らは、MRSA 保菌患者に対する標的除菌(鼻腔ムピロシンとクロルヘキシジンによる全身清拭・シャワー)の有効性を、香港の大学病院で実施した準実験研究により評価した。MRSA 保菌患者 1,928 例を対象に、除菌導入前後を比較した結果、除菌群ではその後の MRSA 感染リスクが 23%有意に低下した(調整ハザード比 0.77)。MRSA 菌血症も 48%低下する傾向を認めたが、有意差には至らなかった。ICU に限定せず病院全体で実施可能な現実的な感染対策として、標的除菌が MRSA 感染予防に有効であることを示した一方、単施設・非ランダム化研究であるため、結果の一般化には慎重な解釈が必要である。
