第 1 波・第 2 波時の急性期医療環境での SARS-CoV-2 による表面および空気汚染★★

背景
医療施設の表面および空気は、重症急性呼吸器症候群コロナウイルス-2(SARS-CoV-2)によって汚染される可能性がある。以前に、筆者らは新型コロナウイルス感染症(coronavirus
disease 2019)パンデミックの第 1 波時(2020 年 4 月)に、筆者らの病院の表面および空気中で SARS-CoV-2 RNA を同定した。
目的
SARS-CoV-2 による表面および空気汚染の特性が 2020 年 4 月から 2021 年 1 月の間に変化したかどうか調査すること。
方法
本研究は、ロンドンの複数施設を有する病院 1 施設における前向き横断観察研究であった。2021 年 1 月に、2020 年 4 月にサンプリングした区域(6 つの臨床区域と公共区域から成る)に相当する区域から表面および空気サンプルを採取した。SARS-CoV-2 は逆転写 PCR およびウイルス培養を用いて検出した。サンプリングは、自然換気のみの 2 病棟でも実施した。本研究の実施時に流行していた変異株の乾燥表面での生存能力を評価した。
結果
生存ウイルスは、表面からも空気からも回収されなかった。270 の表面サンプルの 5%(N = 14)と 27 の空気サンプルの 4%(N = 1)が SARS-CoV-2 陽性であり、これは 2020 年 4 月よりも有意に低かった(218 の表面サンプルの 52%[N = 114]と 27 の空気サンプルの 48%[N = 13]が SARS-CoV-2 陽性、P < 0.001、Fisher の直接確率検定)。SARS-CoV-2 RNA 陽性の表面および空気サンプルの割合に、病棟の換気の種類に基づいた明白な差は認められなかった。試験したすべての変異株が乾燥表面上で 72 時間を超えて生存し、生存ウイルス数の減少は <3-log10 であった。
結論
本研究は、感染予防策の強化によって医療施設の表面および空気への SARS-CoV-2 RNA の負荷が減少したことを示唆している。
監訳者コメント:
強化された感染対策は、患者および職員のユニバーサルマスキング、入院患者全員のスクリーニング、ベッド間隔の拡大 フィジカルディスタンス、週 2 回の迅速抗原定性検査、手指衛生と PPE 着脱の適切な実施、ワクチン接種、訪問者面会禁止、環境清掃頻度の増加と換気であるが、特に高頻度接触面の清掃回数の増加と窓開けによる自然換気の導入は環境表面および空気中のウイルス検出率を有意に低下させた可能性がある。核酸増幅検査では環境表面と空気中のウイルス汚染レベルを推定できるが、ウイルス分離ができておらず、これらのウイルスの感染性については評価することができておらず、標準化されたウイルスのエアサンプリングからの回収実後の培養手技は標準化されておらず、検出方法に問題のある可能性も否定できない。
