背景

カルバペネム耐性腸内細菌目細菌(CRE)保菌者のサーベイランスは、病院感染制御の最初の方策である。CRE 保菌のスクリーニングは、培養および分子技術により評価でき、いずれの方法も所要時間、感度および特異度について独自の特性を有している。

 

方法

本研究は、イタリアのジェノヴァにある 1,200 床の大学病院における前向き研究であり、直腸スワブによる培養結果を解析して CRE スクリーニングを実施した。18 か月にわたる介入において、陰性結果の報告後に、対応するプレートの培養時間を 48 時間から 288 時間に延長して、培養に及ぼし得る影響を評価した。

 

結果

患者計 362 例から得たスワブ 19,278 個と、対応するプレートを対象とした。ベースラインの培養後、プレートの陽性率は 3%であった一方、全体的な培養時間の延長後には 3.7%になった。培養時間の延長は、最も代表的な微生物種の相対頻度の変化と関連した。特に、肺炎桿菌(Klebsiella pneumoniae)の全株および耐性株の存在の減少(P < 0.001)およびエンテロバクター・クロアカエ(Enterobacter cloacae)複合体の全株および感受性株の存在の増加(P < 0.001)が認められた。培養時間の延長により、腸内細菌目細菌の全 ertapenem 高度耐性株(MIC ≧ 4 mg/L)(P = 0.005)、特に肺炎桿菌(P < 0.001)の検出頻度も低下した一方、ertapenem 耐性が低い微生物においては E. cloacae 複合体の存在が増加した(P < 0.001)。

 

結論

培養時間の延長により、回収された CRE の菌数は増加し、耐性の低い分離株と、その他の検出されなかった菌種の回復によって、直腸保菌での細菌シナリオが拡大した。

監訳者コメント

培養時間 48 時間から 288 時間に延長して便でのカルバペネム耐性腸内細菌目細菌の陽性率を 3%から 3.7%に改善した研究。288 時間の培養は臨床的には現実的ではないが、48 時間の培養時間では見逃しが多いことがわかる。
