集中治療室患者におけるカテーテルへの細菌定着およびカテーテル由来血流感染に対する消毒薬含浸カテーテルの有効性 ★

本研究は、重症患者におけるカテーテルへの細菌定着およびカテーテル由来血流感染の発生率に対して、クロルヘキシジンおよびスルファジアジン銀を含浸する中心静脈カテーテルが与える影響を評価するために実施した。中心静脈栄養を必要とする患者133例を、消毒薬を含浸するトリプルルーメン・カテーテル(64例)または標準的なトリプルルーメン・カテーテル(69例)にランダムに割り付けた。カテーテル留置平均日数および標準偏差は、消毒薬含浸カテーテル11.7(5.8)日(中央値10、範囲3~29)、標準的カテーテル8.9(4.6)日(中央値8.0、範囲3~20)であった(P=0.006)。消毒薬含浸カテーテルの14例(21.9%)と標準的カテーテルの14例(20.3%)で、抜去時に細菌定着が認められた(P=0.834)。カテーテル由来血流感染は、消毒薬含浸カテーテル4例(6.3%)、標準的カテーテル1例(1.4%)で発生した(P=0.195)。カテーテルへの細菌定着率は、消毒薬含浸カテーテル群で18.7/1,000カテーテル・挿入日、標準的カテーテル群で22.6/1,000カテーテル・挿入日であった(P=0.640)。カテーテル由来血流感染の発生率は、消毒薬含浸カテーテル群5.3/1,000カテーテル・挿入日、標準的カテーテル群1.6/1,000カテーテル・日であった(P=0.452)。結論として、著者らの結果は、重症患者における消毒薬含浸中心静脈カテーテルの使用は、カテーテルへの細菌定着率とカテーテル由来血流感染のいずれの発生にも影響を与えなかったことを示唆している。
