医療従事者の共感度との関連でみた「手指衛生の 5 つの機会」に関する観察された遵守と自己報告による遵守の差★★

背景

重要な時点における手指衛生(世界保健機関のモデル「手指衛生の 5 つの機会」により確立されている)は、依然として医療関連感染症のリスクを低減するための主要な指標である。多くの介入法が医療従事者の手指衛生遵守を改善することが検証されている一方、手指衛生遵守と医療従事者の共感度との関係についてはほとんど知られていない。

 

目的

機会別にみた手指衛生遵守率と、医療従事者の共感度との関係について、個人レベルと病棟レベルの両方で検討すること。

 

方法

手指衛生遵守のデータを観察および自己報告によって収集し、共感度レベルを確立された質問票によって測定した。本調査は、ドイツの 3 次病院 3 施設の 38 病棟で実施した。観察データは、調査前または後に最長 8 か月間実施した院内観察により収集した。

 

結果

医療従事者の共感度と機会別にみた手指衛生遵守率との間に予想された相関のエビデンスが、観察された手指衛生遵守率(機会 1:r = 0.483、P = 0.031、機会 2:r = 588、P = 0.006)および自己報告による手指衛生遵守率(機会 1:r = 0.093、P = 0.092、機会 2:r = 0.145、P = 0.008)の両方に認められた。分散分析では、共感度(すなわち、低い共感度と高い共感度の比較)および指定された手指衛生の時点(すなわち、基準タスクの前後での比較)の間に認められた決定的な交互作用も有意であった。

 

結論

機会別にみた手指衛生遵守率の差を説明する際に、医療従事者の共感度を、重要な因子として考慮すべきである。したがって、共感度は、質の高い患者ケアにとっての重要な因子であるのみならず、手指衛生遵守の改善における重要な貢献因子とみなされることは明らかである。

監訳者コメント

人の常として、自分自身に危険がおよぶか、そうでないかにより、行動は大きく変わる。手指衛生の 5 つのタイミングも、「患者に触れた後、体液曝露の後、患者環境に触れた後」の3 つは、自分に危険がおよぶ可能性があるので当然遵守率は高くなり、一方で、患者に関連する「無菌的・清潔処置の前、患者に触れる前」は遵守率が低くなる傾向がある。この論文ではこの人の共感度について初めて明らかにした論文であり、「~の前」の手指衛生遵守率に絞ったアプローチが今後必要となる。
