パンデミック時の歯科、口腔／顎顔面外科治療における COVID-19 の伝播：日本の大学病院 51 施設における質問票調査★

背景
新型コロナウイルス感染症（COVID-19）パンデミックは、公衆衛生上の大きな問題となっている。エアロゾルを発生させる歯科処置は、感染リスクを高めると考えられる。したがって、歯科医師や歯科衛生士など歯科医療従事者は、ウイルス伝播のリスクが高い可能性がある。しかし、歯科治療の環境での COVID-19 クラスターについて報告した研究はほとんどない。
目的
歯科、口腔／顎顔面処置が COVID-19 クラスターの発生と関連するか否か、また歯科クリニックでの院内感染の予防のために講じられている対策について調査すること。
方法
複数の大学病院の歯科、口腔／顎顔面外科部門の各施設を対象に、臨床活動（管理的制御）、感染制御策（環境／技術的制御、個人防護具など）、患者・臨床スタッフの COVID-19 確定例または高可能性例に関して、オンライン質問票調査を実施した。
結果
51 施設の職員が質問票に回答した。全職員が歯科および口腔外科外来患者の治療に従事し、標準的予防策を積極的に実施した。14 施設では、職員が COVID-19 患者を治療したが、これらの患者から医療従事者への感染伝播は認められなかった。7 施設では、患者が治療後に感染していることが認められたが（医療従事者が濃厚接触であった）、患者から医療従事者への伝播はなかった。4 施設では、医療従事者が感染していたが、その医療従事者から患者への感染伝播は起こらなかった。
結論
適切な予防策を実施すれば、歯科および口腔外科ケアの環境において COVID-19 クラスターが発生する可能性は低い。

日本の大学病院 51 施設の歯科、口腔外科における診療行為と COVID-19 感染に関する質問票調査である。回答率 79.7％であり、回収率の高い調査結果となっている。調査票では、具体的な感染対策の実施状況をこまやかに確認しており、特に情報が不足しがちである歯科領域を含め、自施設の対策を考える際に大変有用である。
