Detection of influenza virus in air samples of patient rooms

背景
病院環境や医療施設でのエアロゾルを介するインフルエンザウイルスおよび RS ウイルスの伝播と拡散について理解することは、予防策の開発に向けて重要である。
 

方法
2017 年 から 2018 年のインフルエンザシーズン中に、インフルエンザまたは RS ウイルス感染症の確定患者を登録した。病室の空気サンプルを患者の頭部に近い位置
（0.30 m）と頭部から離れた位置（2.20 m）で採取した。空気サンプル中のウイルス粒子の検出と定量のために、リアルタイム PCR を用いた。検出されたウイルスの感染の確定にはプラークアッセイを用いた。
 

結果
検査でインフルエンザが確定した 29 例の病室から 51 個の空気サンプルを採取し、そのサンプルの 51％がA 型インフルエンザウイルス陽性であった。 A 型インフルエンザウイルス陽性患者の 65％がウイルス排出者（少なくとも 1 個の空気サンプルが陽性）で、これは非ウイルス排出者と比較して院内伝播リスクが高いことを反映している。A 型インフルエンザウイルス陽性の空気サンプルの大半（61.5％）が、患者の頭部から 0.3 m で採取されたが、残りのこのウイルス陽性の空気サンプルは、患者の頭部から 2.2 m で採取された。空気サンプルの A 型インフルエンザウイルス陽性率は、患者の頭部からの距離、入院後のサンプル採取日により影響を受けた。RS ウイルス感染症患者の登録は 3 例のみで、いずれの患者もウイルス排出者ではなかった。
 

結論
インフルエンザウイルスは病室の 1 m を超える範囲でエアロゾル化される可能性あり、これは感染制御策により安全とみなされる距離である。エアロゾル化されたウイルス粒子の感染が及ぶ範囲を明らかにするために、さらなる研究が必要である。

インフルエンザ陽性者のいる病室の空気サンプルを患者の頭部に近い位置（0.30 m）と頭部から離れた位置（2.20 m）で採取しウイルス量を測定した論文である。インフルエンザでは、2.2メートル離れた場所のサンプルでもウイルスが検出されていたことは、今後の感染対策を考えるうえで換気の重要性を示すものと思われた。
