バンコマイシン耐性腸球菌(VRE)感染症の予防は医療上の優先課題である。しかしながら、VRE 制御介入の費用対効果は不明である。本研究の目的は、VRE 制御策、例えば、スクリーニング、接触予防策、患者隔離などの経済的評価に関するエビデンスを統合することである。1985 年 1 月から 2018 年 6 月に英語で発表された文献を検索し、病院環境での VRE 制御策の経済的評価に関するものを対象とした。計 4,711 報の論文を抽出し、9 報の主要な研究が、設定した基準に合致した。すべての研究で何らかの VRE スクリーニング法および接触予防策について評価され、対象は院内の 1 病棟(または選択患者集団)から多数の医療施設まで多岐にわたる集団であった。使用された介入と比較に著しいばらつきがあった。研究の大半(7 報)が費用対効果分析を実施し、2 報は費用結果分析であった。すべての経済的評価が病院の視点からであった。4 報の研究では、VRE に特有の制御策強化の実施により、費用効果的/費用削減となることが示され、2 報の研究では、VRE に特有の制御策の中止は費用効果的ではないないことが示された。3 報の研究では、VRE に特有の制御策の漸減により、費用効果的/費用削減となることが示された。Joanna Briggs Institute(JBI)の経済的評価チェックリストに基づくと、対象とした研究の質は概して低く、重要な限界として、介入効果推定に関するバイアスリスク、感度分析の欠如などがあった。ほとんどの研究で、何らかの VRE スクリーニング法と接触予防策の使用が費用効果的であることが示されている。研究の質が低いことと、介入と比較対照の不均一性により、VRE に特有の制御介入の費用対効果について明確な結論が得られていない。有用なエビデンスを強化するために、さらに質の高い経済的評価が必要である。

バンコマイシン耐性腸球菌(VRE)感染症の対策についての経済的評価のレビューである。対象となった論文は2001年から2016年までに公開された9報であった。対策の実施、対策の軽減について双方有用とする論文があり、医療機関の背景による可能性を考察していた。
